男男性接触人群的自我效能对艾滋病知识行为影响分析

摘　　要：目的研究男男性行为人群的一般自我效能对艾滋病知识行为影响,探索解决该人群"知行分离"的干预策略。方法采用应答者驱动(RDS)和网络招募调查对象,收集一般自我效能感、艾滋病防治知识、行为、HIV感染状况资料,用SPSS软件进行单因素和多因素分析。结果被调查者一般自我效能感较低。知识知晓率达84.79%,但最近一次同性肛交安全套使用率68.01%,安全套坚持使用率36.76%,HIV感染率2.99%。多因素分析显示,坚持使用安全套的影响因素为艾滋病防治知识得分、自我效能评分和年龄(OR=1.62、2.06和0.95)。自我效能评分越高,其最近一次同性行为时安全套使用率越高、HIV感染率越低。自我效能评分的影响因素为文化程度、月收入、本地MSM朋友数量、年龄,标准化回归系数分别为0.21、0.16、0.12和-0.13。结论自我效能感同时对艾滋病防治知识和安全套使用行为起到重要的促进和转化作用,提示增加自我效能是解决男男性行为人群"知行分离"的有效策略之一。Objective To research the impact of self-efficacy on knowledge and behavior about AIDS and to explore the intervention strategies to solve ＂separation of knowledge and behavior＂ among men who have sex with men （MSM）.Methods The research objects were recruited by respond driven sampling （RDS） and intemet recruitment.The data of general self-efficacy,AIDS prevention knowledge and behavior,and HIV infection status were collected.SPSS software was used for univariate and multivariate analysis.Results The average self-efficacy scores of the MSM respondents in Xiangyang City was lower than the norm（2.35-± 0.49 vs 2.86 ± 0.51,P＜0.01）.The awareness rate was 84.79％.The condom use rate during the last sexual behavior was 68.01％ and the persistent consistent condom use rate was 36.76％.The HIV infection rate was 2.99％.Multivariate analysis showed that the awareness scores,self-efficacy scores and age were the influencing factors of persistent condom use,with the odds ratio （OR） values of 1.62,2.06 and 0.95 respectively.The higher the self-efficacy score was,the higher the condom use rate during the last sexual behavior and the lower the HIV infection rate were.Education level,monthly income,the number of local MSM friends and age were the influencing factors of the self-efficacy scores,with the standardized regression coefficients of 0.21,0.16,0.12 and-0.13 respectively.Conclusions Self-efficacy score plays an important role in promoting AIDS prevention knowledge and behavior and condom use among MSM.Increasing self-efficacy can be an effective strategy to settle ＂separation of knowledge and behavior＂ among MSM.
