摘  要:目的探讨临床分离的对亚胺培南耐药的鲍曼不动杆菌耐药性和相关的耐药基因。方法收集42株对亚胺培南耐药的鲍曼不动杆菌。采用纸片扩散法测定亚胺培南耐药菌株对18种抗菌药物的敏感性,采用PCR扩增碳青霉烯酶基因,超广谱β-内酰胺酶基因以及氨基糖苷类酶基因。结果 42株对亚胺培南耐药的鲍曼不动杆菌对阿米卡星耐药率最低(28.5%);42株对亚胺培南耐药的鲍曼不动杆菌有33株携带OXA-23组基因,aac(3)-Ⅰ21株,aac(3)-Ⅱ1株,aac(6′)-Ⅰb15株,ant(3")-Ⅰ20株,ant(2")-Ⅰ38株,PER15株,TEM124株,未检测到其他耐药基因。结论耐亚胺培南鲍曼不动杆菌的耐药现象非常严重,OXA-23型是最主要的碳青霉烯酶,TEM、PER是最主要的β-内酰胺酶,aac(3)-Ⅰ、aac(3)-Ⅱa、acc(6′)-Ⅰb、ant(3")-Ⅰ、ant(2")-Ⅰ是最主要的氨基糖苷类酶。Objective To identify the antibiotic resistance and the carbapenemases genotypes of imipenem-resistant strains of Acinetobacter baurnannii. Methods 42 imipenem-resistant strains of Acinetobacter baumanii were isolated. The drug suseeptivity of imipenem-resistant strains to 18 antimicrobial agents were determined by K-B method. The genotypes of carbapenamases were de- termined by PCR. Results The resistant rates of 42 isolates of Acinetobacer baumannii to amikacin were the lowest(28.5%). 33 isolates were positive for OXA-23 gene. Conclusion The phenomenon oF resistance in imipenem-resistant strains of Acinetobacter baurnannii is serious. OXA-23-1ike genes are the most popular carbapenemases coding genes. TEM and PER are the most popularβ- lactamases. The enzymes of aac(3)- I ,aac(3)- 1I a,acc(6r)- I b,ant(3")- I ,and ant(2")- I are the most popular aminoglycosides modifying enzymes.
