益气血补肝肾中药对辅助生殖技术胚胎质量的影响

摘　　要：目的:观察益气血补肝肾中药周期辨证施治对辅助生殖技术胚胎质量的影响及其临床疗效。方法:收集2010年5月1日至2011年4月30日在我院辅助生育中心就诊的符合纳入标准的不孕症患者253例,随机分为中药试验组(116例)、单纯西药对照组(137例)。对照组按常规法予辅助生殖技术(ART)治疗,中药组在常规治疗的基础上给予中药干预。结果:中药周期辨证施治能降低ART促卵泡刺激素(FSH)的周期用量,提高正常受精率、优质胚胎率、种植率、妊娠率,减少获卵率、优质胚胎空泡形成及早期流产的发生,与常规行ART相比差异有显著性(P<0.05);成熟卵率、卵裂率、生化妊娠率、优质胚胎不均质、融合情况未见明显差异(P>0.05);在预防并发症-卵巢过度刺激综合征方面尚未见明显差异(P>0.05)。结论:运用益气血补肝肾中药周期辨证施治于ART中,与单纯西药组相比,中药能与促排卵西药一起发挥协同作用,减少FSH用量,从而减少过多卵子的生成,能提高正常受精率;减少胚胎碎片及空泡形成,降低早期流产率,进而提高种植率与妊娠率。Objective To observe the clinical effect and function of embryo quality of patients received the dialectical cycle of TCM therapy based on syndrome differentiation to ART. Methods Two hundred and fifty-three infertile women in the assisted reproduction center of our hospital from May 1, 2010 to April 30,2011 were randomly enrolled into two groups, the chinese medicinal herbs group （n = 116） and the control group （n = 137）. Patients in the control groupreceived the conventional therapy-IVF/ICSI-ET, and the patients in another group received the chinese medicinal herbs. Results Compared with the control group with the conventional IVF-ET therapy, the cycle therapy of TCM could reduce the cycle dose of follicle stimulating hormone during ART, with significant improvement of normal fertilization rate, excellent embryo rate, implantation rate , pregnancy rate, and reducing the rate of oocytes,the formation of vacuoles in excellent embryo and the early abortion （P 〈 0.05, respectively）. No obvious changes in mature egg rate, rate of ovum cleavage, biochemical pregnancy rate,the situation of uneven cytoplasm , cell fusion and the prevention of OHSS were found between these two goups（P 〉 0.05, respectively）. Conclusion Compared with the conventional methods,the use of cycle therapy of TCM in ART andthe chinese medicinal herbs with conventional methods for ovulation could make a synergistic effect, reducing the dose of little follicle stimulating hormone, inhibiting the excessive eggs formation, improving nthe ormal fertilization, reducing the formation of fragment and vacuoles in embryo, decleasing the rate of early abortion, and ultimately improving the implantation rate and pregnancy rate.
