中药口服配合灌肠治疗溃疡性结肠炎的临床观察及对肠黏膜IL-6、IL-10的影响

摘　　要：目的观察理肠汤口服结合溃疡灵灌肠治疗溃疡性结肠炎(UC)的临床疗效,并检测患者结肠黏膜组织中的IL-6、IL-10的表达情况。方法将UC患者60例随机分为治疗组30例与对照组30例,治疗组给予理肠汤口服及溃疡灵灌肠;对照组给予虎地胶囊口服,两组均治疗2个月。治疗结束后进行临床疗效比较,并利用免疫组化的方法,检测结肠黏膜组织中IL-6、IL-10在治疗前后的表达。结果两组治疗后均能改善腹泻、脓血便、腹痛等主要临床症状(P<0.05),治疗组优于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05);两组均能降低IL-6的阳性表达率、提高IL-10的阳性表达量(P<0.05),治疗组优于对照组(P<0.05)。结论理肠汤口服结合溃疡灵灌肠治疗UC有较好的临床疗效,能够明显改善患者症状,减轻肠黏膜炎症,降低肠黏膜IL-6的表达、升高IL-10的表达。Objective： To observe the clinical efficacy of orally taken Li-Chang decoction combined with Kuiyangling enema on ulcerative colitis. Method： 60 cases of ulcerative colitis patients were randomly divided into 2 groups： treatment group and control group. Treatment group was given Li-Chang decoction orally and Kui- Yang-Ling enema;the control group was given Hu-di capsule;Both groups were treated for 2 months. The clinical ef fects were compared after treatments. By using immunohistochemistry,we detect two groups of patients with colonic mucosa tissue IL-6, IL-10 expression in the changes before and after treatment, and comparison. Results： The treatment group and the control group can improve clinical symptoms such as diarrhea,bloody mucopurulent stool and abdominal pain and other （P〈 0.05）, and the treatment group clinical curative effect was better than the control group （P〈 0.05）. Both of them can decreased the positive expression of IL-6,increase the rate of IL-10 positive expression, The treatment group decreased positive expression of IL-6 in colonic mucosa and improve positive expression of IL-10 was better than the control group （P〈 0.05）. Conclusion： Oral Li-Chang decoction combined Kui-Yang-Ling enema has been proved a prominent clinical curative effect in treating ulcerative coli- tis,which could significantly improve clinical symptoms,reduce the intestinal mucosal inflamma tory response, decrease the content of IL-6,and increase IL-10 content. Oral Li-Chang decoction combined Kuiyangling enema can adjust the immune function,inflammatory cytokines and inflammation cell inhibitor y factor,so as to achieve to the equilibrium state.
