摘 要:目的探讨慢性血吸虫病患者结肠腺咬合蛋白(occludin)与紧密连接蛋白-1(ZO—1)的表达及其临床意义。方法选取50例慢性血吸虫病患者及9例正常结肠黏膜标本,采用免疫组织化学(SABC)染色法,检测occludin与ZO-1在结肠腺上皮细胞的表达情况。结果与对照组比较,慢性血吸虫病组结肠腺结构不完整、腺体萎缩;慢性血吸虫病组结肠腺上皮细胞occludin与ZO-1阳性颗粒平均灰度值(159.26±6.57、166.07±5.44)明显高于对照组(155.53±5.90、162.16±5.14,P〈0.05),阳性单位值(37.27±2.63、34.69±2.19)明显低于对照组(38.85±2.42、36.37±2.11,P〈0.05);慢性血吸虫病组结肠腺occludin与ZO-1表达量的减少与性别及年龄无关(P〉0.05)。结论血吸虫感染可以损伤结肠腺上皮细胞间的紧密连接蛋白,从而破坏患者结肠黏膜屏障的完整性。Objective To study the expression of occludin and zonula occludens-1 (ZO-1) in patients with chronic schistosomiasis and its clinical significance. Methods Colonic mucosa specimens of 50 patients with chronic schistosomiasis and 9 cases of normal people (as a control group) were selected and the expressions of occludin and ZO:I in colonic epithelial cells were determined using immunohistochemical staining(SABC). Results Compared with the normal control group, the structure of colonic gland was incomplete with glandular atrophy, mean gray values(159.26 ~ 6.57, 166.07 ~ 5.44) of the positive particles of oceludin and ZO-1 of colonic epithelial cells in the chronic schistosomiasis group were significantly higher than those of the normal control group(155.53 ~ 5.90, 162.16 ~ 5.14, P 〈 0.05), but positive unit values(37.27 ~ 2.63, 34.69 ~ 2.19) were significantly lower than those of the control group(38.85 ~ 2.42, 36.37 ~ 2.11, P 〈 0.05); reduced expression of oecludin and ZO-1 in colonic gland of the patients with chronic schistosomiasis had nothing to do with sex or age (P 〉 0.05). Conclusions Infection of schistosome can reduce the expression of oecludin and ZO-1 which can damage the tight junction between colonic epithelium cells, thereby undermining the integrity of the colonic mueosa barrier.
