辽宁省城市布鲁杆菌病的再现及原因分析

摘　　要：目的了解辽宁省城市布鲁杆菌病（简称布病）的再现情况，并分析其发生原因。方法2010、2011年，对辽宁省网上直报的城市户籍的新发布病病例进行复核诊断[包括布病试管凝集实验（SAT）和布病病原体分离实验1：对辽宁省城、乡人间布病新发病例数进行汇总分析；对城市新发布病病例进行接触史调查。结果共报告城市布病新发病例57例，追踪核实诊断24例，SAT检验结果均为阳性，其中2例患者的静脉血中分离到羊种Ⅲ型布鲁杆菌。2011年城市和农村的新发病例数（36、817）明显高于2010年（21、566），而农村与城市发病比呈现下降趋势（2010、2011年分别为26：1、22：1）。24例城市新发病例均有牛羊肉接触史，无人传人现象发生。结论辽宁省城市人间布病已再现，呈散发状态，通过皮肤感染是导致城市疫情产生的重要原因。Objective To investigate the epidemic status of brucellosis in urban of Liaoning Province and to analyze the reason for its reoccurrence. Methods Diagnosed new cases that were directly reported via internet in 2010 and 2011 were re-checked [using brucellosis tube agglutination test （SAT） and pathogen separation test ]. The incidence of human brucellosis was analyzed and contact history of new cases was investigated in urban and rural areas of Liaoning Province. Results Fifty-seven new cases of brucellosis were reported in the cities and 24 of them were verified. All patients were positiye in SAT test, and a type m brucellosis bacillus was isolated from vein blood of two patients. The number of new cases in urban and rural areas in 2011（36, 817） was significantly higher than that in 2010（21, 566）. But the incidence ratio of rural areas showed a downward trend compared to that of urban areas（26 ： 1 and 22 ： 1 in 2010 and 2011 , respectively）. All the 24 new cases had a history of exposure to beef and mutton via skin, but no human-to-human infection occurred. Conclusions Brucellosis has reappeared in urban of Liaoning Province in a sporadic state. An important reason is that the infection is passed through skin.
