摘  要:目的 探讨椎间盘突出和游离的MRI表现特点和鉴别诊断.方法 分析40例椎间盘突出和游离MRI表现的部位、信号、增强表现、硬膜囊和脊髓受压情况.结果 40例中,椎间盘突出33例,椎间盘游离7例.T1WI突出和游离的椎间盘呈等信号,T2WI呈低信号.2例增强扫描呈环形强化.突出和游离的椎间盘对硬膜囊均有明显压迫,3例颈髓受压变性.结论 MRI可准确显示椎间盘突出和游离,对制订手术治疗方案有重要价值.Objective To investigate MRI findings and differential diagnosis of intervertebra disc herniation and free prolapse nucleus pulposus. Methods Characteristics of MRI findings of 40 cases of patients with intervertebra disc herniation and free prolapse nucleus pulposus were retrospectively analyzed, including the sites, signals, enhanced MR manifestations and the degree of compression of dural sac and spiral cord. Results There were 33 cases of intervertebral disc herniation, 7 cases of free prolapse nucleus pulposus. The signal intensity of the herniated discs and free nucleus were isointensity on T1WI and hypointensity on T2WI. 2 cases showed ring-enhancement. Both the herniated discs and free nucleus oppress the dural sac and spiral cord. In 3 cases, there were cervical cord degeneration due to compression. Conclusion MRI can display accurately the intervertebrai disc herniation and free prolapse nucleus pulposus,it has important clinical value for clinical treatment.
