HIV感染者外周血中调节性T细胞研究

摘　　要：目的研究人类免疫缺陷病毒(human immunodeficiency virus,HIV)感染者外周血中调节性T细胞(regulation T cell,Treg)和辅助性T细胞(helper T cell,Th)水平的变化,目的是分析这种变化与疾病的进展关系。方法我们采用流式细胞术对HIV感染组和正常对照组外周血中的Treg及T淋巴细胞亚群进行检测,采用CD4+CD25+CD127-/low及CD4+CD25+2种方式标记Treg,比较分析HIV感染者Treg水平与正常人群的差异,回归分析HIV感染者的Treg水平与CD4+、CD8+T绝对值变化的相关性。结果结果显示HIV感染者CD4+CD25+CD127-/lowTreg/CD4+T、CD4+CD25+Treg/CD4+T显著高于正常对照组(P<0.01);另外,HIV感染者CD4+CD25+CD127-/lowTreg/CD4+T与CD4+T绝对值变化显著负相关(r=-0.718,P<0.01),与CD4/CD8比值显著负相关(r=-0.710,P<0.01),与CD8+T绝对值无相关性(r=-0.137,P>0.05);HIV感染者CD4+CD25+Treg/CD4+T与CD4+T绝对值变化无相关性(r=-0.199,P>0.05),与CD4/CD8比值负相关(r=-0.475,0.05>P>0.01),与CD8+T绝对值正相关(r=0.495,0.05>P>0.01)。结论 HIV感染者Treg/Th平衡性明显向Treg方向倾斜,而且HIV感染者CD4+CD25+CD127-/lowTreg/Th平衡性变化与疾病进展关系密切,随着疾病的进展而向Treg方向倾斜,可作为疾病进展监测的指标之一;CD4+CD25+Treg/Th平衡性变化与疾病进展关系不明确。We aimed to investigate the balance between regulation T cells （Treg） and helper T （Th） cells in the peripheral blood of HIV-infected people, and analyze the relationship between the change of balance and disease progress. Firstly, we tested Treg and Th cells in the peripheral blood of HIV-infected people and normal people by use of flow cytometry, and marked Treg by two means （CD4＋CD25＋CD127-/Low and CD4＋CD25＋）. Then we analyzed the balance of Treg/CD4＋T in HIV-infected people and normal group, and analyzed the relevance between Treg level and CD4＋T or CD8＋T cell counts. We found that CD4＋CD25＋D 127-/LowWTreg/CD4＋Tand CD4＋CD25＋Treg/CD4＋T levels in HIV- infected people were significantly higher than that of normal control （P〈 0.011）. We also found that there was significant negative correlation between CD4＋2D2532D127--/LowWTreg/CD4＋ T and the CD4＋T cell counts in HIV-infected people, and significant negative correlation between CD4＋CD25 ＋CD127--/Low Treg/CD4＋ T and CD4/CD8 too, but there was no correlation between CD4＋CD25＋CD127-/LowWTreg/CD4＋T and CD8＋T cells. In addition, CD4＋CD25＋Treg/CD4＋T in HIV- infected people did not correlate with CD4＋T cell counts, but negatively correlated with CD4/CD8, and positively correlated with CDS＋T cells. In conclusion, the balance between Treg and Th cells in HIV-infected people changes obviously, and slopes to Treg cells. Furthermore, with the disease advancing the balance between CD4＋D25＋CD 127-/Low Treg and Th cells is changing, and slopes to Treg cells. So the balance can be used to monitor the progress of HIV. However, CD4＋CD25＋Treg/CD4＋T cell is not suitable to be used to monitor the progress of HIV.
