微创小切口治疗胫骨平台骨折的疗效观察

摘　　要：目的比较微创经皮钢板内固定(MIPPO)与切开复位钢板内固定方法治疗胫骨平台骨折的临床效果。方法回顾性研究2006年1月—2012年8月该院收治的胫骨平台骨折46例,其中微创经皮钢板内固定(M组)21例,切开复位钢板内固定(S组)25例,分别采用微创经皮钢板内固定(M组)与切开复位钢板内固定(S组)方法治疗,术后随访11个月至1年半。比较两组关节功能优良率、住院时间、骨折愈合时间及并发症。结果与S组比较,M组平均住院时间和平均骨折愈合时间明显降低,S组平均住院时间17.5 d,平均骨折愈合时间15.5周,术后感染和局部皮肤坏死各1例,关节僵硬2例;M组平均住院时间14.8d、平均骨折愈合时间11.2周,无深部感染、关节僵硬、皮肤坏死及关节畸形等并发症发生。术后随访12个月,M组HSS评分为91.5分(78～95分),优16例,良3例,中2例,优良率90.5%;S组平均分79.8分(61～88分),优15例,良5例,中5例,优良率80%。比较两组平均住院时间、平均骨折愈合时间和HSS评分具有统计学意义(P<0.05)。结论微创经皮钢板内固定(MIPPO)治疗胫骨平台骨折能降低平均住院时间和骨折愈合时间,提高膝关节功能恢复,是治疗胫骨平台骨折的理想方法。Objective To compare the efficacy of minimally invasive percutaneous plate the fixed （MIPPO） and open reduction and internal fixation for tibial plateau fractures. Methods Retrospective study of 46 patients with tibial plateau fractures admitted to Haushan People＇s hospital during January 2006 to August 2012 was conducted. Group M （21 cases） underwent minimally invasive percutaneous plate osteosynthesis （MIPPO） while group S （25 cases） underwent open reduction and internal fixation. After the surgery, patients were followed up ranging from 11 months to 18 months. The excellent rate of outcomes, hospital stay and bone recovery time as well as inci- dence of complications of the two groups were compared. Results For group S, the average hospital stay was 17.5d, bone recovery time was 15.5 weeks, and the complication included infection and topical skin necrosis respectively observed in 1 case, and joint stiffness in 1 case. For group M, the average hospital stay was 14.8 d, bone recovery time was 11.2 weeks, and no complication occurred in all cases. The average HSS scoring of M group was 91.5 points （from 78 to 95 points）, and the excellent rate was 90.5% including excellent in 16 cases,good in 3 cases,moderation in 2 cases;In S group,the average HSS scoring was 79.8 points （from 61 to 88） and the excellent rate was 80% including excellent in 15 cases ,good in 5 cases and moderation in 5 cases. There were significant differences in the average hospital stay, bone recovery time and the average HSS scoring between the two groups. Conclusions MIPPO reduces the average hospital stay and bone recovery time, and improves knee function recovery. Therefore, it is a safe and effective procedure for the tibial plateau fractures.
