摘  要:目的:分析在心瓣膜置换术患者心脏复跳前行主动脉根部灌注硝酸甘油对手术疗效的影响。方法:筛选96例心脏瓣膜疾病患者,随机分为两组,均行心瓣膜置换术。试验组48例患者术中应用硝酸甘油行主动脉根部灌注。对照组48例患者按照常规处理,不使用硝酸甘油灌注。观察两组心脏自动复跳率、多巴胺药物使用量等指标。结果:试验组心脏自动复跳率97.91%高于对照组66.67%,差异有统计学意义(P<0.01)。试验组肾上腺素使用量、多巴胺使用量、心脏复跳后CPB(体外循环)辅助时间均少于对照组,差异均有统计学意义(P<0.05)。结论:在心瓣膜置换术中心脏复跳前主动脉根部灌注硝酸甘油并且配合心肌保护措施,可明显提高心脏自动复跳率,促进术后恢复作用明显。Objective: To analyze the efficacy of nitroglycerin perfusion via the aortic root before heart resuscitation for patients undergoing heart valve replacement surgery.Method: 96 cases of valvular heart disease screened were randomly divided into two groups. Two groups of patients underwent heart valve replacement surgery.48 patients in the test group were injected nitroglycerin in the aortic root. 48 patients in the control group were treated as normal, without using nitroglycerin infusion. The auto-resuscitation heart rate and other indicators of two groups were observed. Result: The auto-resuscitation heart rate of experimental group , which was 97.91%, was higher than that of control group , which was 66.67%, the difference was statistically significant ( P〈0.01 ) .The epinephrine usage, dopamine usage and CPB supporting time after heart re-beating of experimental group were less than those that of the control group, the differences were statistically significant (P〈0.05) .Conclusion: Such measure, nitroglycerin perfusion via the aortic root before heart re-beating for patients undergoing heart valve replacement, in conjunction with other measures of myocardial protection can significantly increase the automatic heart re-beaiing rate, shorten the CPB supporting time after heart resuscitation, and improve postoperative clinical outcomes.
