摘 要:目的探讨扩张性心肌病患者血清尿酸(SUA)水平与功能性二尖瓣返流(FMR)之间的相关性。方法选42例扩张性心肌病住院患者,所有患者均依据纽约心脏病协会心力衰竭分级标准分级(NYHA分级),并于入院第2天测定患者SUA、血浆B型脑钠肽(BNP)水平、心脏射血分数及二尖瓣幕状区面积,计算SUA与二尖瓣幕状区面积之间的相关性。结果在42例扩张性心肌病患者中,随着NYHA评级增高,患者血BNP、SUA水平逐渐升高,二尖瓣幕状区面积逐渐增大,而心脏EF值逐渐降低,其组间差异具有显著性(P<0.01);重度FMR患者SUA水平显著高于轻/中度FMR患者;SUA与二尖瓣幕状区面积显著正相关(r=0.33,P<0.01)。结论扩张性心肌病患者血清尿酸水平与功能性二尖瓣返流具有相关性。Objective To evaluate the relationship of uric acid and functional mitral regurgitation in dilated cardiomyopathy patients. Methods 42 cases with dilated cardiomyopathy were included into this study. All the patients were classified according to NYHA criteria. Serum uric acid (SUA), brain natriuretic polypeptide (BNP), left ventricular ejection fraction (LVEF) and mitral valve tenting area were detected on the 2nd day after the patients hospitalized. Then the relationship of SUA and mitral valve tenting area was evaluated. Results BNP, SUA and mitral valve tenting progressively increased from New York Heart Association (NYHA) class II to IV in all the patients with dilated cardiomyopathy, while LVEF decreased gradually. There were significant differences among the groups ( P 〈 0.01 ). The patients with severe FMR had significant- ly higher SUA levels compared with mild/moderate FMR patients. And a significantly positive correlation were found between SUA and mitral valve tenting area ( r = 0. 33, P 〈 0. 01 ). Conclusion This study demonstrates that elevated SUA levels in dilated cardiomyopathy patients correlated with the severity of FMR.
