小针刀配合龙氏正骨手法治疗颈源性肩周炎疗效观察

摘　　要：目的观察小针刀配合龙氏正骨手法治疗颈源性肩周炎的临床疗效。方法将144例颈源性肩周炎患者随机分为A组、B组和C组,每组48例。A组采用小针刀配合龙氏正骨手法治疗,B组采用针刺配合龙氏正骨手法治疗,C组采用常规针刺配合推拿治疗。比较3组治疗前后疼痛评分、肩关节功能评分及临床疗效。结果 3组治疗后VAS评分与同组治疗前比较,差异均具有统计学意义(P<0.01)。A组治疗后VAS评分与B组、C组比较,差异均具有统计学意义(P<0.01,P<0.05)。A组治疗前后肩关节功能各项评分差值与B组、C组比较,差异均具有统计学意义(P<0.01,P<0.05)。A组总有效率为100.0%,B组为87.5%,C组为83.3%,A组总有效率与B组、C组比较,差异均具有统计学意义(P<0.01,P<0.05)。结论小针刀配合龙氏正骨手法是一种治疗颈源性肩周炎的有效方法。Objective To observe the clinical efficacy of needle knife plus Long＇s manipulations in treating periarthritis of shoulder due to cervical spondylosis. Method Totally 144 patients with periarthritis of shoulder due to cervical spondylosis were randomized into group A, group B, and group C, 48 in each group. Group A was intervened by needle knife plus Long＇s manipulations, group B was by acupuncture plus Long＇s manipulations, and group C was by conventional acupuncture plus tuina. The three groups were compared in pain score, shoulder joint function score： and clinical efficacy. Result The Visual Analogue Scale （VAS） scores changed significantly after treatment in the three groups （P〈0.01）. After treatment, group A was significantly different from group B and group C in comparing VAS score （P〈0.01,P〈0.05）. The changes of shoulder function scores in group A were significantly different from those in group B and C （P〈0.01,P〈0.05）. The total effective rate was 100.0% in group A, versus 87.5% in group B and 83.3% in group C, and the differences were statistically significant （P〈0.01,P〈0.05）. Conclusion Combination of needle knife and Long＇s manipulations is an effective approa：h in treating periarthritis of shoulder due to cervical spondylosis.
