锁定加压钢板治疗老年桡骨远端不稳定骨折

摘　　要：目的探讨锁定加压板在老年桡骨远端不稳定骨折治疗中的应用价值。方法回顾性分析北京市潮白河骨伤科医院2007年1月至2011年6月收治的43例老年桡骨远端不稳定骨折病例，给予锁定加压钢板固定、植骨治疗。其中男14例，女29例；年龄60～79岁，平均68．3岁。骨折采用AO分型，B2型5例，B3型8例，c1型9例，c2型15例，C3型6例。通过术后随访检查腕关节功能恢复情况及X线片表现评定该方法的疗效。结果术后随访6～22个月，平均14．2个月，43例均获得骨性愈合，平均愈合时间8．2周，无畸形愈合。腕关节掌倾角平均8．6。，尺偏角平均18．4。；桡骨短缩6例，均小于2mm。按Dienst功能评估方法进行疗效评价，优17例，良23例，可3例，优良率93．02％。所有患者无神经损伤、无感染等并发症。结论锁定加压钢板固定治疗老年桡骨远端不稳定骨折疗效满意，对恢复并维持桡骨远端高度和关节面平整不失为一种较好的选择。Objective To evaluate the effects of in the locking compression plate （LCP） fixation in treatment of the unsta- ble distal radial fractures in elderly patients. Methods From January 2007 to June 20011,43 patients with unstable distal ra- dial fractures were fixed with anatomical Locking Compression Plates and tamped bone graft. By AO classification, there were 5 cases of type B2,8 cases of type B3,9 cases of type C1,15 cases of type C2 and 6 cases of type C3. The outcomes were as- sessed by radiographs and evaluation of wrist function through the postoperative follow - up. Results Mean follow - up time was 14.2 months （6 to 22 months）. All cases acquired bony union with a mean healing time of 8.2 weeks without malunion. The mean palmar tilt angle was 8.6° （3° to 12° ）. The mean ulnar inclination angle was 18.4° （ 13° to 24°）. There were radial shortening in 6 cases, the mean length was less than 2 mm. According to Dienst Category, there were excellent in 17 cases, good in 23 cases, fair in 3 cases, and the total efficacy rate was 93.02 %. There was no iatrogenic nerve injury and infection. Con- clusion For the unstable distal radial fractures in the elderly patients, Locking Compression Plate fixation has satisfactory cur- ative effect. This procedure can restore and sustain the distal radial length and smooth articular surface after the fracture reduc- tion.
