单纯减压手术与减压并植骨融合治疗中央型腰椎间盘突出症的比较研究

摘　　要：目的比较单纯减压手术与减压并植骨融合治疗腰椎间盘突出症的临床疗效。方法选取2009年1月至2012年1月来我院予以减压并植骨融合治疗的85例中央型腰椎间盘突出症患者为研究组,选取同期来我院予以单纯减压手术治疗的85例中央型腰椎间盘突出症患者为对照组,比较两组治疗后的临床疗效、手术指标及腰背痛、疼痛的变化。结果两组治疗后JOA评分、VAS评分与治疗前比较,差异具有统计学意义(P<0.05)。治疗后研究组JOA评分与对照组比较,差异具有统计学意义(P<0.05),但VAS评分与对照组比较,差异无统计学意义(P>0.05)。研究组的手术时间、术中出血量、住院天数、住院费用均明显高于对照组,差异具有统计学意义(P<0.05)。研究组治疗后总有效率为95.29%,对照组为87.06%,经比较差异具有统计学意义(P<0.05)。结论减压并植骨融合治疗中央型腰椎间盘突出症比单纯减压手术的疗效好,改善腰背痛的效果好,但因其费用高、手术时间长等特点,故在对中央型腰椎间盘突出症患者予以手术治疗时,还需考虑患者的经济因素。Objective To compare the clinical efficacy of simple decompression surgery and decompression combined with interbody fusion in the treatment of lumbar disc herniation clinical efficacy.Methods Eighty-five patients with central lumbar disc herniation in our hospital from January 2009 to January 2012 treated by decompression and interbody fusion were enrolled as the study group.Another 85 patients treated by simple decompression surgery in the same time period were selected as the control group.The clinical efficacy,surgical indicators,low back pain,and the change of pain were compared between the two groups.Results The JOA score,VAS score after treatment of the two groups showed statistically significant difference compared to that before treatment （P〈0.05）.After treatment,JOA score showed statistically significant difference between the two groups （P〈0.05）,but VAS score did not （P〉0.05）.The operation time,blood 1oss,length of hospital stay,hospital costs in the study group were significantly higher than those in the control group （P〈0.05）.The total effective rate was 87.06% in the control group and 95.29% in the study group,with statistically significant difference between the two groups （P〈0.05）.Conclusion Compared with simple decompression surgery,decompression combined with interbody fusion shows higher clinical efficacy in the treatment of central lumbar disc herniation,which helps improve the effect of back pain.However,because of its high cost and long operation time,the economic factors of the patients should be taken into account in the treatment of central lumbar disc herniation.
