成人OSAHS患者的综合治疗策略

摘　　要：目的：探讨成人阻塞性睡眠呼吸暂停低通气综合征（OSAHS）患者的综合治疗策略。方法：2009年8月～2012年12月在无锡市第四人民医院耳鼻咽喉科就诊的136例OSAHS患者，根据中华医学会标准，分为轻、中、重三组，其中轻度47例，中度45例，重度44例。比较各组综合治疗前后ESS评分及最低血氧饱和度（SaO2）之间的变化，同时观察各组综合治疗后的治愈率、显效率、有效率及无效率。结果：①三组患者综合治疗后ESS评分下降，治疗前分别为（7．72±3．58，15．43±2．03，20．17±2．61）分，治疗后为（0．87±1．15，8．75±1．97，11．09±2．15）分，各组治疗前后比较差异有统计学意义（P〈0．01）；②PSG检查评价显示各组患者最低SaO2在治疗前后分别为轻度组（90．23±2．29，95．66±1．16）％；中度组（81．75±3．21，89．34±1．97）％；重度组（63．09±7．34，73．19±4．16）％，各组患者治疗前后最低SaO2比较差异有统计学意义（P〈0．01）。③OSAHS综合治疗策略的治愈率为25．74％，显效率为36．76％，有效率为27．94％，无效率为9．56％。结论：在OSAHS的治疗过程中，应根据病因及病情，综合应用器械、手术、药物和行为治疗等方法，制定综合治疗方案。Objective ：To explore the integrated therapy protocol of adult patients with obstructive sleep apnea - hypopnea syndrome （OSAHS）. Methods：According to the Chinese Medical Association standards, 136 patients with OSAHS from August 2007 to December 2012 in the Department of Otorhinolaryngology, Wuxi Fourth People＇s Hospital, were divided into three groups ： mild group （47 patients）, middle group （45 patients）, severe group （44 patients）. The integrated therapy were taken into individuals in different groups, then compare the different of the assessment of ESS and the minimum value of SaO2. The groups also observed after integrated therapy cure rate, markedly effective rate, efficient rate and inefficient rate. Results：（1）The ESS score of the three groups of patients has decreased after integrated therapy. Before therapy were （7.72 ± 3.58,15.43 ± 2. 03,20. 17 ± 2. 61 ） points, after therapy were （0. 87 ± 1.15,8.75 ± 1.97, 11.09 ±2. 15） points, before and after therapy in each group difference was statistical significant （P 〈0. 01 ）. （2）The lowest SaO2 in each group before and after integrated therapy were mild group （90. 23 ±2. 29,95.66 ± 1.16）% ; moderate group （81.75 ±3.21,89. 34 ± 1.97）% ; severe group （63.09 ±7.34, 73. 19 ±4. 16）%, before and after integrated therapy in each group the lowest SaO2 difference was statistical significant （P 〈0. 01 ）. （3）The OSAHS integrated therapy cure rate was 25.74%, markedly effective rate is 36.76%, efficient rate was 27. 94%, inefficient rate was 9.56%.Conclusions：To treat OSAHS patients more effectively, the integrated therapy protocol with breathing machine, oral appliance, surgery, drug and behavior therapy is necessary, which based on the etiology and the graduate of patients conditions.
