自动痔疮套扎联合外切术治疗重度混合痔的疗效比较

摘　　要：目的探讨自动痔疮套扎联合外切术用于治疗重度混合痔的可行性。方法 212例混合痔患者随机分为传统组和改良组,分别行传统内扎外切术和自动痔疮套扎联合外切术,比较两组患者手术时间、术中出血量和平均住院时间;记录术后30d内并发症的发生率。结果改良组患者手术时间明显短于传统组(P<0.01),术中出血量明显少于传统组(P<0.05),住院时间明显短于传统组(P<0.05),改良组在肛门疼痛、切口水肿、尿潴留等并发症的发生率上明显低于传统组(P<0.05);改良组患者术后3d内VRS评分显著低于传统组(P<0.01)。结论自动痔疮套扎联合外切术用于重度混合痔的治疗可大大节省手术时间和住院时间,同时痛苦轻微,较好地保留了直肠和肛门的正常结构和功能,适合在临床广泛开展。Objective to study the feasibility of the automatic hemorrhoids ligation and resection for severe mixed hemorrhoids. Methods 212 cases of mixed hemorrhoid patients were randomly divided into the traditional group and improved group who respectively received the traditional and automatic hemorrhoids ligation and resection. The operation time, amount of bleeding and hospital stays were compared and the incidence of complications within 30 days after operation was kept.Results The operation time of the improved group was shorter than that of the traditional group （P〈0. 01） and the amount of bleeding was significantly less than that of the traditional group（P〈0. 05）. The hospital stays were shorter than those of the traditional group （P〈 0. 05）. The complications incidence of anal pain, incision edema and urine retention of the im- proved group was significantly lower than that of the traditional group （P〈0. 05）. The VRS scores for the improved groups within 30 days after operation were significantly lower than those of the traditional group （P〈0. 01）.Conclusion The operation time and hospitalization stays can be greatly shorten if automatic hemorrhoids ligation and resection was used for the severe mixed hemorrhoids.Besides,it can ease pain and preserve the normal structures and functions of the rectum and anus. So the automatic hemorrhoids ligation and resection was suitable for the extensive application in clinical practice.
