摘  要:目的观察转移瘤与骨质疏松所致椎体压缩性骨折影像学表现,探讨其鉴别诊断价值。方法收集我院15例确诊恶性肿瘤患者全脊柱MRI及CT表现,老年骨质疏松椎体压缩性骨折患者27例椎体MRI及CT表现,重点观察压缩椎体部位、范围,附件,形态,信号,椎旁软组织,椎间隙。结果转移瘤较特异征象:多发椎体及附件受累;椎体后缘膨隆;膨胀性骨质破坏,椎体及椎旁不规则软组织;椎间盘受累罕见;DWI呈高信号。结论 MRI联合CT检查对压缩性骨折病因诊断有较高准确率,且MRI与CT对比发现MRI更敏感,更能提高病灶检出率。Objective To observe the imaging manifestations of metastasis and osteoporotic vertebral compression fractures and explore the differential diagnosis value.Methods The whole spine MRI and CT from 15 patients with malignant tumor patients and vertebral body MRI and CT manifestations of 27 patients with osteoporotic vertebral compression fractures were collected.The compressed vertebral body parts,range,the attachment,morphology,signal,paravertebral soft tissue,and vertebra clearance were observed.Results The specific signs of metastatic tumor included the involvement of multiple vertebral body and annex,vertebral bombe,expansive bone destruction,irregular vertebral and paravertebral soft tissue,rare intervertebral disc involvement,and high signal showed in DWI.Conclusion MRI combined with CT scan in the etiological diagnosis of compression fractures can improve lesion detection rate.Moreover,compared with CT,MRI is more sensitive in detecting the lesions.
