错过合适年龄段完全性心内膜垫缺损外科矫治疗效分析(附15例报告)

摘　　要：目的评价错过合适年龄段完全性心内膜垫缺损患者的外科矫治疗效。方法对2002年12月至2012年6月15例完全性心内膜垫缺损患者行矫治术。手术方法采用单片法修补房室间隔缺损3例;改良单片法修补房室间隔缺损8例;双片法修补房室间隔缺损4例。同期矫治其他合并畸形。观察病死率及术后并发症等的变化。结果全组手术死亡2例,病死率13.3%;随访8～120个月,1例于术后36个月因晚期妊娠合并二尖瓣重度关闭不全致左心衰竭死亡;余患者生存质量良好。结论错过合适年龄段的完全性心内膜垫缺损患者行外科矫治术,也可能取得满意的临床疗效。Objective To summarize the surgical experience and the results of 15 patients who received complete atrioventricular ca nal operation. Methods Repair of atrioventricular septal defect was performed in 15 patients from December 2002 to June 2012. Three pa tients were performed with single patch technique, 8 patients with simplified single patch technique, and 4 with two patch technique. Other associated malformations were repaired at the same time. The death rate and postoperative complications for these cases were observed. Results Two patients died after the repair of atrioventricular septal defect performed in the group. The overall mortality rate was 13.3%. Follow up 8 120 months showed 1 patient died 36 months after the surgery for sever degree mitral regurgitation accompanied with late stage pregnancy, and all other survivals showed increased physical activities obviously. Conclusion Satisfactory results may be acquired in pa tients of missed the optimal surgery age.
