101例腹膜透析患者心功能评价及意义

摘　　要：目的探讨心脏彩超检查在腹膜透析(PD)患者心功能评价的作用及意义。方法选择101例住院的持续非卧床腹膜透析(CAPD组)患者及30例正常人(对照组)为研究对象,应用心脏彩超测定CAPD患者及对照组左心房内径(LAD)、左室舒张末期内径(LVDd)、左室收缩末期内径(LVDs)、室间隔厚度(IVST)、肺动脉压力(PAP)及舒张早期和舒张晚期二尖瓣口最大血流速度之比(E/A)等,计算左心室质量指数(LVMI)。结果 CAPD组LAD、LVDd、LVDs、IVST、PAP、及LVMI高于对照组(P<0.05),E/A低于对照组(P<0.05);相关性分析显示CAPD组LVEF与BNP呈负相关(r=-0.467,P=0.000);LVMI与BNP(r=0.416,P=0.006)呈正相关。结论心脏彩超可用于CAPD患者心脏结构和功能的评估。Objective To analyze the cardiac function value and the clinical application evaluation of echocardiography in peritoneal dialysis （PD） patients. Methods Left atrial diameter （ LAD）, left ventricular end diastolic diameter （ LVDd）, left ventricular end systolic diameter （LVDs）, interventricular septal thickness （IVST）, interventricular septal thickness （PAP）, left ventricle weight index （LVMI） early and late wave of mitral valve flow （E/A） were determined by echocardiography in 101 patients with continuous ambulatory peritoneal dialysis （CAPD） and 30 normal persons （ control group）. Results Echocardiography showed that LAD, LVDd, LVDs, IVST, PAP, LVMI of CAPD group were higher than those of the control group （P 〈 0.05 ）, while E/A was lower than that of the control group （ P 〈 0.05 ）.Correlation analysis showed that LVEF was negatively correlated with BNP （ r = - 0. 467, P = 0. 000 ） ; LVMI was positively correlated with BNP （r = 0.416,P = 0. 006）. Conclusion Echocardiography can accurately evaluate the cardiac function, which is worth further clinical research.
