辨证分型治疗胃十二指肠溃疡随机平行对照研究

摘　　要：[目的]观察辨证分型治疗胃十二指肠溃疡疗效。[方法]使用随机平行对照方法,将110例门诊患者按随机数字表法分为两组。均予以健康宣教、基础护理、心理引导等临床护理。对照组55例尚未穿孔时,碳酸钙、硫酸镁、次碳酸铋等抑酸,阿托品、复方颠茄片等止痛,做好对胃黏膜的保护治疗;穿孔立即手术。治疗组55例辨证分型治疗。肝胃不和型:柴胡疏肝汤;脾胃虚寒型:黄芩建中汤加香砂六君子汤;胃热型:丹栀逍遥散;血瘀型:分别予以桃仁承气汤和归脾汤。1剂/d,水煎300mL,早晚温服。连续治疗30d为1疗程。观测临床症状、不良反应。连续治疗2疗程(60d),判定疗效。对治疗组50例,对照组38例患者随访120d,观察复发情况。[结果]治疗组治愈29例,有效21例,无效5例,总有效率90.10%。对照组治愈18例,有效20例,无效17例,总有效率69.10%。治疗组疗效优于对照组(P<0.05)。[结论]辨证分型治疗胃十二指肠溃疡效果显著,值得推广。[Objective]Observe the diagnosis and treatment of gastric ulcer treatment.[Methods]Using randomized controlled method,110 cases of outpatients were randomly divided into two groups. Are to be health education,basic care,psychological guidance and other clinical care to ensure that the clinical efficacy has increased steadily. Control group,55 cases of perforation has not yet occurred,the use of calcium carbonate,magnesium sulfate,bismuth carbonate acid suppression therapy,etc.,while using atropine,belladonna tablets and other analgesic compound,good protection of the gastric mucosa treatment;Once perforation occurs,take aggressive surgical treatment,such as gastrectomy. Treatment group 55 cases of diagnosis and treatment.Disharmony of liver and type：Chaihushugan soup;Spleen Deficiency Type：Huangqin Jianzhong soup and Xiangsha Liujunzi;stomach type：Danzhi Xiaoyaosan;stasis：be separately Taoren Chengqi decoction and Jianpi soup. 1 / d,decoction 300mL,morning and evening,warm clothes. 30d is a course of continuous treatment. Observation of clinical symptoms and adverse reactions. Continuous treatment of two courses（60d）,to determine efficacy. The treatment group of 50 patients in the control group of 38 patients were followed 120d,observed recurrence.[Results]treatment group were cured 29 cases,effective in 21 cases,5 cases,the total efficiency of 90.10%. The control group were cured 18 cases,effective in 20 cases,17 cases,the total efficiency69.10%. Treatment group than the control group（P0.05）.[Conclusion]Diagnosis and treatment of gastric ulcer effect is significant,worthy of promotion.
