利湿清热活血汤与活络消肿汤熏洗联合黄连膏、铁箍散膏外敷治疗产后血栓性深静脉炎40例临床观察

摘　　要：[目的]观察利湿清热活血汤与活络消肿汤熏洗联合黄连膏、铁箍散膏外敷治疗产后血栓性深静脉炎疗效。[方法]采用前瞻性设计方法,对40例产后血栓性深静脉炎住院患者使用①利湿清热活血汤（金银花、土茯苓、野菊花各30g,防已20g,坤草30g,水蛭、地龙各20g,丹参30g,黄芪40g,牛膝30g,穿山甲10g,泽泻、川芎各20g）,1剂/d,水煎300mL,早晚口服。②活络消肿汤（乳香,没药各10g,木瓜15g,伸筋草、透骨草各30g,红花、大黄各15g）,水煎熏洗,2次/d,30~60min/次,皮肤溃破者忌用。③红肿热痛使用黄连膏外敷,下肢肿伴色紫暗有硬结使用铁箍散膏外敷。连续治疗15d为1疗程。观测临床症状、不良反应。连续治疗2疗程,判定疗效。[结果]痊愈25例,显效9例,有效5例,无效1例,总有效率为97.50%。[结论]利湿清热活血汤与活络消肿汤熏洗联合黄连膏、铁箍散膏外敷治疗产后血栓性深静脉炎疗效满意。[Objective]To observe the treating postpartum thrombotic deep phlebitis 40 cases with Lishi Qingre Huoxue Decoction and Tongluo Xiaozhong Decoction fumigation plus huanglian gao,schisandra propinqua plaster apply effect.[Methods]Adopt forward-looking design method,40 cases of hospitalized patients with postpartum deep thrombotic phlebitis using ①Lishi Qingre Huoxue Decoction（Jinyinhua 30g,Tufuling 30g,Yeju hua 30g,Fangyi 20g,kuncao 30g,Shuizhi 20g,Dilong 20g,Dancan 30g,Huangqi40g,Niuxi 30g,Chuanshanjia 10g,Zexie 20g,Chuanxiong 20g）1 agent /d,water simmer in water 300mL,sooner or later oral.②Tongluo Xiaozhong Decoction（Ruxiang 10g,Meiyao 10g,Mugua 15g,Shenjincao30g,Tougucao 30g,Honghua 15g,Dahuang 15g）,Water frying fumigation,2/d,30 ~ 60min/,skin ulceration jiyong.③Swelling heat pain using external Coptis cream,lower extremity edema with dark purple color with induration use schisandrapropinqua plaster.Continuous treatment with 15d for 1 courses. Observation of clinical symptoms,adverse reaction. Continuous 2 courses of treatment,the curative effect.[Result]25cases were cured,9 cases markedly effective,5 cases,1 cases ineffective,the total effective rate was 97.50%.[Conclusion]Treating postpartum thrombotic deep phlebitis 40 cases with Lishi Qingre Huoxue Decoction and Tongluo Xiaozhong Decoction fumigation plus huanglian gao,schisandra propinqua plaster apply has a good effect.
