穴位埋线治疗不随意运动型脑瘫随机平行对照研究

摘　　要：[目的]观察穴位q埋线治疗不随意运动型脑瘫疗效。[方法]使用随机平行对照方法,将60例住院及门诊患儿按就诊顺序编号法随机分成两组。常规康复治疗:①运动疗法:Bobath法及Vojta法。②按摩。③作业疗法。④物理疗法:a水疗,b蜡疗,c小儿经络导平治疗仪。对照组30例传统针刺经穴法,取肩髃、曲池、外关、八邪、秩边、居髎、伏兔、足三里,1次/d。治疗组30例穴位埋线,取风池次、天髎次、臑会次、天宗次、承山次和伏兔次,1次/10d。连续治疗1个月为1疗程,观测临床症状、粗大运动功能、肌张力波动、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组显效16例,有效8例,无效6例,总有效率为80.00%。对照组显效5例,有效15例,无效10例,总有效率为66.67%。临床疗效治疗组优于对照组(P<0.05)。粗大运动功能均有改善(P<0.01,P<0.05),治疗组优于对照组(P<0.05)。缓解肌张力波动均有改善(P<0.05),治疗组优于对照组(P<0.05)。[结论]穴位埋线治疗不随意运动型脑瘫能显著提高不随意运动型小儿脑瘫的粗大运动功能,缓解肌张力波动,值得在临床上广泛推广。[Objective]Discuss effective treatment for children with involuntary sports cerebral palsy.[Methods]Using random parallel control method,60 cases inpatient and outpatient is divided into two groups at random. Contrasted group of 30 cases use routine rehabilitation therapy in traditional acupuncture meridian method,acupuncture and take outside Jian Yu,Qu Chi,Wai Guan,Ba Xie,Chihpien,Fu Tu,Tsusanli,1time every day,20 days is 1 course of treatment,breaks up ten days before next treatment,a total of 3 period of treatment. Treatment group 30 cases use routine rehabilitation therapy combined with acupoint threadembedding therapy,embedded wire points,take the wind pool ci,Tian Liao ci,Er Hui ci,Cheng Shan Ci,and Fu Tu Ci. Once every ten days,and one month as one course of treatment,while break up 2 days before next course of treatment,continuous for 3 months. Observation of extensor and flexor muscle tension muscle,tendon reflex,asymmetric posture,muscle control,and so on and so forth,gross motor function test scale（GMFM）overall percentage,muscle tension fluctuation change. Judge curative effect.[Result]Treatment Group has16 remarkable valid,8 valid,and 6 invalid,efficiency is 53.3%. Contrasted Group has 5 remarkable valid,15 valid,and 10 invalid,efficiency is 53.3%. Treatment Group is better than contrasted group（P﹤0.05）;The total GMFM percentage increases,muscle tension fluctuations relieve（P0.05,P0.01）,Treatment Group is better than contrasted group（P0.05,P0.01）.[Conclusion]Based on conventional rehabilitation treatments combined with acupuncture point bury a line therapy,it can significantly improve children＇s involuntary sports cerebral palsy gross motor function,relieve muscle tension fluctuation,which is worth widely popularizing in clinic.
