摘 要:目的检测心房颤动患者血浆高敏C-反应蛋白(high sensitive C-reactive protein,hs-CRP)和白介素-6(interleukin-6,IL-6)水平,探讨炎症反应在不同类型房颤中的变化程度及可能的临床意义。方法入选患者143例,其中房颤组患者共98例(阵发性房颤54例,持续性房颤44例),对照组45例。用酶联免疫法检测血浆hs-CRP和IL-6水平并进行比较。结果房颤组患者与对照组患者相比,血浆hs-CRP水平(33.39±33.94mg/Lvs.26.44±32.41mg/L,p=0.03)及IL-6水平(2.85±2.50pg/mlvs.1.17±1.22pg/ml,p<0.01)明显升高。亚组分析显示,持续性房颤组血浆hs-CRP水平(39.91±35.62mg/L vs.26.44±32.41mg/L,p<0.01)及IL-6水平(3.08±2.79pg/ml vs.1.17±1.22pg/ml,p<0.01)均较对照组明显升高;持续性房颤组与阵发性房颤组患者相比较,血浆hs-CRP水平明显升高(39.91±35.62mg/L vs.29.24±44.64mg/L,p<0.01);血浆IL-6水平也有升高趋势,但未达统计学差异(3.08±2.79pg/ml vs.2.25±1.41pg/ml,p=0.44);阵发性房颤组与对照组相比,血浆IL-6水平明显升高(2.25±1.41pg/ml vs.1.17±1.22pg/ml,p<0.01),hs-CRP水平有升高趋势,但未达统计学差异(29.24±44.64mg/L vs.26.44±32.41mg/L,p=0.37)。结论房颤患者血浆炎症因子水平明显升高,其中持续性房颤患者炎症因子升高水平较阵发性房颤患者更明显,提示患者血浆炎症因子升高水平可能与房颤持续时间相关。Objective To investigate the role of inflammation in different categories of atrial fibrillation (AF). Methods A toal of 98 consecutive patients with AF was enrolled in this study. AF was categorized as paroxysmal AF (n=54) and persistent AF (n=44). Another 45 patients with paroxysmal supravemricular tachycardias or ventricular premature constructions consisted of control group. Plasma levels of high sensitive C-reactive protein (hs-CRP) and interleukin-6 (IL-6) were measured by ELISA. Results Both plasma hs-CRP levels (33.39 ± 33.94mg/L vs. 26.44 ± 32.41rag/L, p=O.O3) and IL-6 levels (2.85 ±2.50pg/ml vs. 1.17 ± 1.22pg/mI, p〈O.Ol) in overall AF group were higher than those in control group. In subgroup analysis, plasma hs-CRP levels (39.91 ± 35.62mg/L vs. 26.44 ±32.41mg/L, p〈0.01) and IL-6 levels (3.08 ± 2.79pg/ml vs. 1.17 ± 1.22pg/ml, p〈O.01) wel signilicantly higher in persistent AF group when compared with the control group. Compared with paroxysmal AF group, plasma hs-CRP levels in persistent AF group were higher (39.91 ±35.62mg/L vs. 29.24±44.64rag/L, p〈0.01). The plasma IL-6 levels in paroxysmal AF group were higher than the control group (2.25 ± 1.41 pg/ml vs. 1.17 ± 1.22pg/ml, p〈O.O 1). Although it was not statistically significant, the plasma IL-6 levels in persistent AF group and the plasma hs-CRP levels in paroxysmal AF group were higher than which in paroxysmal AF group and which in the control group respectively. Conclusions Inflammatory biomarkers in AF patient were higher. Furthermore, the levels of inflammatory factors maybe reflect the duration of AF partly.
