军事应激对部队官兵胃黏膜病变的影响

摘　　要：目的对军事应激状态下部队官兵的胃黏膜病变情况进行调查。方法采用消化内窥镜、光学显微镜及快速尿素酶试验检测某部60名官兵军事演习前后的胃黏膜形态、组织病理学改变及幽门螺杆菌(Helicobacter pylori,Hp)感染情况。结果军演后官兵内镜下糜烂性胃炎、胆汁反流性胃炎及十二指肠球部溃疡较军演前增加,并新出现了胃溃疡及急性胃黏膜病变(P<0.01);组织活检标本的光镜结果显示军演后官兵胃黏膜急性炎症改变增多,出现溃疡者胃黏膜上皮破坏严重,细胞界限不清、坏死脱落;胃黏膜Hp感染率在军演前后无明显变化(P>0.05)。结论军事应激可导致官兵胃黏膜病变。重视部队官兵的心理健康,提高应激能力,有助于减少军事应激对于官兵胃黏膜的不良影响。stress. Methods Objective To investigate the pathological changes of gastric mucosa in the troops during military The appearance, histopathological changes and Helicobacter pylori （Hp） infection of gastric mucosa in 60 soldiers before and after military maneuver were examined by upper gastrointestinal endoscopy, light microscopy and rapid urease test. Results The number of erosive gastritis, bile reflux gastritis and duodenal bulb ulcer in the troops after military maneuver was significantly higher than that before military maneuver, and several cases of gastric ulcer and acute gastric mucosa lesion newly came up after military stress （P〈0. 01）. The microscopic result from gastric mucosal biopsy specimen showed that acute inflammatory changes increased after military maneuver. And severe epithelial damage, ob- scure border and necrosis appeared in those newly induced gastric ulcer. However, the rate of Hp infection did not show significant difference before and after military maneuver （P〉0. 05）. Conclusion Military stress can lead to the patholog ical changes of gastric mucosa. Paying attention to the troops mental health and improving their ability to manage physio-logical stress would contribute to reduce the adverse effects of military stress on the gastric mucosa in the troops.
