摘 要:目的观察抗氧化剂α-硫辛酸对2型糖尿病(T2DM)合并下肢动脉粥样硬化的疗效。方法将60例T2DM合并下肢动脉粥样硬化随机分为实验组(30例)和对照组(30例)。对照组给予常规控制血糖治疗,实验组在常规治疗基础上加用硫辛酸0.6 g、1/d,共用药3周。观察两组治疗前、后血糖、血脂、高敏C反应蛋白(hs-CRP)、内皮素(ET-1)、一氧化氮(NO)水平。结果两组治疗前、后血糖、血脂差异无统计学意义(P>0.05)。对照组治疗后hs-CRP、ET-1、NO水平与治疗前比较差异无统计学意义(P>0.05),实验组hs-CRP、ET-1较治疗前下降(P<0.05),NO较治疗前升高(P<0.05);两组治疗后hs-CRP、ET-1、NO差异均有统计学意义(P<0.05)。结论硫辛酸可通过改善内皮功能、抑制炎症反应,在T2DM合并下肢动脉粥样硬化的治疗中发挥重要作用。Objective To observe the effect of antioxidant α-lipoic acid (ALA) in treatment of patients with type 2 diabetes mellitus (T2DM) complicated with lower extremity arteriosclerosis. Methods A total of 60 patients with T2DM complicated with lower extremity arteriosclerosis were randomly divided into experimental group (n = 30) and con- trol group (n = 30). The control group was given routine treatment of hypoglycemic agents, and the experimental group received 0.6g of ALA 1 time/d by intravenous drip infusion based on control group therapy. The two groups were treated for 3 weeks. The levels of blood glucose, blood fat, high sensitive C reactive protein (hs-CRP) , endothelin ( ET-1 ) and nitric oxide (NO) in the two groups were observed before and after treatment. Results The differences in levels of blood glucose and blood fat in the two groups before and after treatment were not statistically significant ( P 〉 0.05 ). There were not significant differences in levels of hs-CRP, ET-1 and NO before and after treatment in control group (P 〉 0.05) ; but in experiment group, levels of hs-CRP and ET-1 were significantly decreased (P 〈 0.05) and NO level was significantly higher after treatment than those before treatment ( P 〈 0. 05 ). The differences in levels of hs-CRP, ET-1 and NO in the two groups after treatment were statistically significant (P 〈 0.05). Conclusion ALA can inhibit inflam- matory reaction by improving endothelial function, and it plays an important role in treatment of T2DM complicated with lower extremity arteriosclerosis.
