妊娠高血压疾病胎盘微血管密度的研究

摘　　要：目的探讨妊娠高血压疾病胎盘微血管密度情况。方法应用免疫组织化学法检测各试验组及对照组胎盘CD 31表达情况,并计算其微血管密度。试验组为轻度子痫前期、重度子痫前期、子痫、HELLP综合征孕妇胎盘各30例;对照组为正常足月妊娠孕妇胎盘29例。应用HE染色了解胎盘组织病理改变。结果轻度子痫前期组微血管密度为(219.33±35.52)与对照组胎盘(233.75±39.56)比低,无明显统计学差异;但重度子痫前期组为(154.25±49.9)、妊娠子痫组为(126.67±43.57)、HELLP综合征组为(158.32±40.12)与对照组胎盘比明显减少(P<0.01),具有显著统计学差异;重度子痫组与轻度子痫组比(P<0.01),妊娠子痫组与重度子痫组(P<0.05),均具有统计学差异。病理学表现:妊高症胎盘终极绒毛数及间质血管减少,合体滋养细胞结节明显增多,贫血性梗死,血管内可见血栓。结论胎盘微血管密度降低、血管形成减少与妊娠期高血压疾病密切相关,并随着疾病的进展其微血管密度降低。Objective To investigate the condition of micorvascular density in hypertensive disorder complicating pregnancy. Methods We detect the expression of CD 31 and caculate the microvessel density （MVD） in cases of normal pregnancy and exprimental pregnancy with immunohistochemistry.The exprimental group included each 30 cases of mild pre-eclampsia,severe pre-eclampsia, eclampsia,HELLP syndrome; the normal group included 29 cases of full-term pregnancy placenta. The pathological changes of placenta tissuse were observed by haematoxylin-eosin staining. Results MVD of mild pre-eclampsia（219.33±35.52）was lower than that of the normal （233.75±39.56）,which wasn＇t significant statistical difference;but that of severe pre-eclampsia （ 154.25±49.9 ）, eclampsia （126.67±43.57）, HELLP syndrome（158.32±40.12） reduced abviously comparing with the normal（P〈0.01）; severe pre-eclampsia was compared with mild pre-eclampsia（P〈 0.01 ）, eclampsia was compared with severe pre- eclampsia（P〈0.05）;flle difference is statiscally significant.Pathological changes ： the number of villi and vessel reduced, syncytiotrophoblast nodules were significantly increased, anemic infarct, intravascular thrombus was observed. Conclusion Lower MVD in placenta, the reduce of formation of vessels are closely related to hypertensive disorder complicating pregnancy ,and the microvascular density is lower with disease developing.
