经桡动脉介入术导致前臂血肿处理方法的比较研究

摘　　要：目的 比较经桡冠脉介入诊疗术后患者出现前臂张力性血肿时弹力绷带压迫和绷带压迫联合脱水处理两种方法的优势和不足,为临床治疗提供依据.方法 回顾146例冠脉介入术后出现前臂张力性血肿患者,采用单独绷带压迫（A组）和绷带压迫联合脱水（B组）等方法处理后,对血肿成功进行处理的病例运用t检验进行统计学分析,比较两种处理方法的效果差异.结果 B组76例前臂张力性血肿患者与A组70例患者比较,前臂肿胀消退平均时间明显缩短,手指末端的血供和功能改善为良好的出现时间明显缩短,差异均有统计学意义（P＜0.05）;周围皮肤温度增高出现的平均时间稍有缩短（P＞0.05）,在出现肿胀压迫后3h观察前臂平均臂围,相对于患者未穿刺血管前臂臂围稍有缩小,但差异无统计学意义（P＞0.05）.结论 经桡动脉介入术后导致前臂张力性血肿,在弹力绷带压迫基础上积极进行脱水等综合处理疗效更好,方法简便,疗效确切、可靠、安全.Objective Through the comparison analysis, these two methods of elastic bandages and bandage compression with dehydration are found to its advantages and disadvantages in application, when the forearm tensition hematoma be occurred because of transradial coronary intervention （TRI）, and to provide the ideal method of scientific treatment when the hematoma were found. Methods The forearm tensity haematomas after pereutaneous coronary interventional operation in 146 patients were analyzed retrospective- ly, a bandages （group A） and （B group）bandage compression with dehydration were used when the haematoma were processed, and the T-test was applied statistically. Results In the 146 patients of fore- ann tensity haematoma, （B group）the 76 patients were given in compression by the bandage compression and dehydrated, when compared with A group, the time of forearm subsiding was shortened obviously （P〈 0.05）, the differences has statistics significance ; The average time the swelling subsided and the time of fin- ger in blood supplying and function were reduced obviously（P〈0.05）, the skin temperature had risen slightly （P〉0.05）, difference has not statistics significance; the arm circumference of forearm has slightly reduced after 3 hours of compression when compared with normal upper limb （P〉0.05）, the difference was not statis- tically significant. Conclusion The bandage plus dehydration treatment is a safe and practical and reliable alternative approach, when the forearm tensity haematoma were caused after the transradial coronary intervention.
