摘 要:目的 探讨床上康复操护理干预对肾移植患者术后早期康复的影响.方法 将126例肾移植手术患者随机分为康复操组和对照组各63例.两组均进行常规治疗及护理,康复操组从术后24h开始由护士指导并协助患者每天进行2次自编床上康复操训练.应用功能综合评定量表(FCA)对两组患者每日运动功能(自我照料、转移、行走能力)进行评分,并统计两组患者住院期间并发症的发生率.结果 术后第1天康复操组FCA总分为(32.87±1.76)分,高于对照组的(30.22±1.39)分,至术后第5天康复操组FCA评分为(59.13±1.27)分,仍高于对照组的(47.10±1.24)分,两组患者FCA总分的变化经重复测量方差分析,差异具有统计学意义(F=994.67,P<0.01).康复操组下床时间为(3.00±0.89)d,对照组为(5.00±0.58)d,两组比较差异有统计学意义(t=-3.261,P<0.01).康复操组并发症发生率与对照组差异无统计学意义(P>0.05).结论 床上康复操有利于提高肾移植患者术后自我照料、转移、行走能力,缩短患者卧床时间,降低并发症的发生率.Objective To explore the effect of on-bed rehabilitation exercise on the early rehabilitation of patients after renal transplantation. Methods One hundred and twenty-six patients with renal transplantation were randomly divided into the rehabilitation exercise and the control group, each with 63 cases. Both groups were performed conventional treatment and nursing. The patients in rehabilitation exercise group were administrated rehabilitation exercise twice per day from 24 hours after operation by the nurses to guide and assist. The daily movement functions including self-care, transfer and walking ability were assessed by function comprehensive assessment (FCA), and the incidence rate of complication was analyzed during hospitalization in both two groups. Results On the first day after operation, the FCA score of the rehabilitation exercise group was (32. 87 ± 1.76), which was significantly higher than that of the control group (30. 22 ± 1.39). On the fifth day after operation, the FCA score of the rehabilitation exercise group was (59. 13 ± 1.27), which was significantly higher than that of the control group (47. 10 ± 1.24 ), and the difference of FCA scores change of two groups with by repetitive measure analysis of variance was statistically significant (F =994.67 ,P 〈0. 01 ). Getting out of bed time in the rehabilitation exercise group was ( 3.00 ± 0. 89 ) d, which was significantly shorter than that of the control group ( 5. 00 ± 0. 58 ) d, and the difference was statistically significant ( t = - 3.261, P 〈 0. 01 ). The difference in the incidence rate of complication was not statistically significant between two groups ( P 〉 0. 05 ). Conclusions On-bed rehabilitation exercise is helpful to improve self-care, transfer and walking ability in patients after renal transplantation, shorten the time lie in bed, and reduce the incidence rate of complication.
