选择性贲门周围血管离断术治疗肝硬化门静脉高压症的临床分析

摘　　要：目的：研究分析临床应用选择性贲门周围血管离断术治疗肝硬化静脉高压的效果。方法：选取2007年7月-2009年4月在我院诊治的186例肝硬化静脉高压症患者，按随机数字表法随机分为对照组和观察组各93例，观察组患者给予选择性贲门周围血管离断术进行治疗，对照组患者应用常规手术方法进行治疗，观察两组患者治疗后的临床效果。结果：术后观察组患者的FPP、门静脉血流量、肝动脉血流量、白细胞、血小板、总胆红素改善程度明显大于对照组（P〈0.05）；两组并发症发生率及复发率比较，观察组明显低于对照组（P〈0.05），有统计学意义。结论：临床应用选择性贲门周围血管离断术治疗肝硬化静脉高压效果明显，能有效的减少术后并发症及再次出血的情况发生。Objective：To observe and investigate the clinical therapeutic effects of selective pericardial devascularization on curing liver cirrhosis with portal hypertension.Methods：186 cases of liver cirrhosis with portal hypertension,hospitalized in our clinic over July,2007 to April,2009,were randomly assigned to observation group or control group according to random number table.Each group was consisted of 93 cases respectively.Selective pericardial devascularization was performed on each case in observation group,meanwhile conventional devascularization was applied in control group.Therapeutic effects were analyzed in both groups.Results：The FPP,portal venous blood flow,hepatic arterial flow,leukocyte account,platelet account and total bilirubin of observation group were improved significantly than those of control group （P〈0.05）.The occurrence rate of complications and the recurrence rate in observation group were lower than those in control group,and the differences were statistically significant （P〈0.05）.Conclusion：The clinical therapeutic effects of selective pericardial devascularization on curing liver cirrhosis with portal hypertension were evident,and the occurrence of postoperational complications and rebleeding was significantly decreased.
