儿童慢性胃炎患者^(13)C-尿素呼气试验和胃黏膜活体组织检查Hp检测的比较研究

摘　　要：目的:对慢性胃炎儿童患者13C-尿素呼气试验(13C-UBT)和胃黏膜活体组织检查(活检)Hp两种检测方法进行比较分析。方法:选取我院收治的慢性胃炎儿童患者46例,分别进行13C-尿素呼气试验检测和胃黏膜活检Hp检测,并以胃黏膜活检Hp检测结果为标准,比较13C-UBT对幽门螺旋杆菌检出的准确度、特异性和敏感度;并对检测出Hp阳性患者采用质子泵抑制剂(PPI)进行治疗,观察其临床疗效。结果:胃黏膜活检Hp检测结果显示,13C-UBT的准确度、特异性和敏感度分别为97.83%、92.86%和100%,且对Hp阳性患者采用PPI三联疗法治疗的转阴率要显著优于采用单纯PPI和PPI二联疗法的患者,差异有统计学意义(P<0.05)。结论:与胃黏膜活检Hp方法相比,13C-UBT方法在Hp的检出上具有较高的准确度、特异性和敏感度,同时操作简单、对人体无害和非侵入性等优点,是儿童Hp感染慢性胃炎的理想诊断方法,值得在儿科推广和应用。Objective：To children chronic gastritis patients 13 c-UBT and gastric mucosal biopsies Hp two kinds of detection methods comparative analysis.Methods：The patients of chronic gastritis,46 cases of children were 13 c-urea breath test and gastric mucosal biopsies Hp testing,more 13 c-UBT for helicobacter pylori detection accuracy,specificity and sensitivity,And the detection of Hp positive patients treated by PPI and to observe its efficacy.Results：The gastric mucosal biopsies Hp testing results showed that 13 c-UBT accuracy,specificity and sensitivity were 97.83%,92.86%and 100%,and the PPI triple therapy,shade rate was superior to pure PPI and PPI duplex therapy,significant difference （P〈0.05）.Conclusion：And gastric mucosal biopsy method than Hp,13 c-UBT method in Hp＇s detection has higher accuracy,specificity and sensitivity,and at the same time,the operation is simple,harmless to the human body and non-invasive etc.
