摘 要:目的为了观察血沉这项临床检验指标在临床的辅助诊断价值。方法采用回顾性研究分析临床血液标本血沉检测结果和临床诊断的关系。结果在血沉正常范围内,本院最常见的患者临床诊断为心脏病,随着血沉检测值的增加,患者类型发生变化。炎症患者和肿瘤患者血沉极度增快的比例高。单因素分析老年人和中青年患者资料,骨骼系统疾病的血沉极度增快差异有统计学意义(P<0.01)。血沉极度增快和血沉值不超过50 mm/h的肿瘤,高血压,心脏病和有炎症患者的差异具有统计学意义(P<0.01)。结论血沉检测不适用于患有基础疾病如心脏病,高血压和糖尿病的辅助诊断,此三者临床检测以正常范围或低于50 mm/h的居多,但可用于鉴别诊断。血沉极度增快的患者炎症和肿瘤的风险高,临床意义较大。Objective To observe the clinical assistant value of the erythrocyte sedimentation rate (ESR). Methods Retro- spective analysis of ESR was carried out to reveal the relationship between ESR and clinical diagnosis. Results Within the nor- mal reference of ESR, heart disease was most common among patients, and the types of patients changed with SER increase. The rapid increase of ESR was frequent in patients with inflammation and tumors. ESR increase difference was statistically sig- nificant in bone disease between elderly patients and young patients( P 〈 0.01 ). There was significant statistical difference a- mong patients with tumor, hypertension, heart disease and inflammation, when the ESR was below 50 mm/h(P 〈 0.01 ). Con- clusion ESR was not a good assistant tool for diagnosis of base diseases, such as heart disease, hypertension and diabetes mel- litus, as they were mostly found with normal ESR or ESR under 50 mm/h, but a good indicator for differential diagnosis. The patients with extreme elevation of ESR had high risk for inflammation and tumor.
