北京地区HBsAg阳性母亲的儿童乙型肝炎疫苗免疫后血清学监测结果分析

摘　　要：为了评价母亲HBsAg阳性儿童乙型肝炎 (乙肝 )疫苗免疫效果 ,寻求加强免疫时机 ,北京市卫生防疫站于 1996～ 1999年连续 4年对 1～ 5岁儿童免疫后效果进行流行病学、血清学的横断面调查。结果显示 :5岁以下儿童抗 -HBs阳性率为 78 6 6 %～80 5 6 % ,抗 -HBs阳性率随年龄增长呈下降趋势 ;5岁以下儿童HBsAg携带率为 3 41%～ 3 5 1% ,未呈现随年龄的增长而上升的趋势。不同地区儿童免疫后效果较一致 ;母亲HBsAg阳性儿童接种乙肝疫苗阻断效果理想 ,儿童HBsAg携带率主要为宫内感染或出生后早期感染。据此建议对该类儿童应在出生后 1～The aim of this study was to identify the effect of hepatitis B vaccine in children born to HBsAg positive mothers and looked for an proper time for enhanced immunization Children(1～5 years old)born to HBsAg positive mothers were immunized at birth with HB vaccine and a cross-sectional study for four successive years(1996～1999)was carried out with seroepidemiololgical survey The results showed that the anti-HBs positive rate of children kept 78 54%～80 56% during the five years since immunization,and the rate declined along with age increasing (P<0 01) The HBsAg carrier rate was 3 41%～3 51% in them,it did not rise along with age (P>0 05) The anti-HBs positive seroconversion rate of city children (83 42%) was higher than that of the suburb children (P<0 01) and the HBsAg carrier rate in the former was lower (1 58%) than that of the latter (P<0 01) A similar immune effect was seen in children born to HBsAg positive mothers either in districts or in counties of Beijing The perinatal transmission of HBV can be blocked by the vaccine. The carrier rate of HBsAg of the children mainly attributes to uteral infection or early-stage infection after birth A suggestion of implementing an enhanced HB vaccination to these children was put forward
