上海市彭浦街道老年慢性病人就医行为的研究

摘　　要：目的　了解 6 0岁以上人群患者就医行为及影响因素 ,为改善社区卫生服务提出建议。方法　病史询问、实验室检查及询问调查相结合。结果　调查 980名 6 0岁以上老人 ,发现高血压 ,高血脂 ,高血糖 ,肥胖症及心电图异常患者 833人 ,患者有 84. 8%接受社区卫生服务中心及服务站诊治 ,通过社区医生判断上述患者 ,94%可在社区卫生机构接受诊治 ,6 %患者需转至 2～ 3级医院治疗 ;患者住院有 5 3 . 9%愿意选 2 - 3级医院 ,而社区医生判断有 5 3 . 2 %患者可以在家庭病床及一级医院接受住院治疗。结论　从政策上调整病人流向 ,使 80 %以上门诊病人及二分之一以上住院患者接受社区卫生服务是必要的 ,可行的。Objective To study the seeking health care behavior of the elderly in Pengpu street for purposes of improving community health care services.Methods 980 cases of the elderly over sixty were interviewed with laboratory examination and medical history collecting.Results Among them,833 cases with hypertension high cholesterol,dialetes obesity and EKG abnormal were interviewed with seeking health care behavior.84.8% patients actually accepted health care at community health center and doctors thought that only 6% patients needed to be hospitalized at large hospital.Conclusion It is necessary and feasible for over 80% out-patients and half of in-patients accept the community health services by adjustment of medical policy.
