摘  要:目的研究ABCB1基因单核苷酸多态位点G2677T/A在中国汉族人群中的分布及其与氯吡格雷抵抗(elopidogrelresistance,cR)的相关性。方法患者人选后均给予氯吡格雷600mg和阿司匹林300mg负荷量治疗,并在服药前和服药后24h分别测定5vmol/L腺苷二磷酸诱导的血小板聚集率(PAR),根据2次测定结果计算PAR变化值,PAR变化值小于或等于10%者定义为CR。采用焦磷酸测序法测定ABCB1基因G2677T/A单核苷酸多态位点在CR组和NCR组的基因型及等位基因分布频率。结果ABCB1基因G2677T/A多态性在CR组和NCR组基因型分布频率符合HaZy—Weinberg平衡。在CR组和NCR组中,ABCB1基因G2677T/A基因型频率分布差异无统计学意义(P〉0.05);两组G、T、A等位基因的分布频率差异也无统计学意义(P〉0.05,OR=0.81—1.31,95%C/=0.38—4.2)。结论ABCB1基因单核苷酸多态位点G2677T/A与冠心病患者CR的发生无相关关系。Objective To investigate the frequency of gene polymorphism in G2677T/A of ATP - binding cassette subfamily B member l transporter (ABCB1) gene and the correlation between this polymorphism site and the occurrence of clopidogre] resistance (CR). Methods Patients selected were treated with loading doses of 600 mg clopidogrel and 300 mg aspirin. Platelet aggregation ratio (PAR) induced with 5 μmol/L ADP was assessed before and after clopidogrel administration at 24 h. An absolute reduction of PAR ( ≤ 10% ) compared with baseline was defined as CR. Genotype was determined by polymerase chain reaction (PCR) and the sequence of G2677T/A ofABCB1 gene was analyzed. Results The genotype frequencies in G2677T/A of ABCB1 gene conformed well to the Hardy - Weinberg equilibrium for both the CR group and the NCR group. No significant difference in frequencies of the genotype and allele existed in two groups. Conclusions Based on the genetic analysis in patients after PCI with clopidogrel therapy, there is no correlation between ABCB1 G2677T/A polymorphism and elopidogrel resistance.
