急性心肌梗死患者早期室性心律失常与血钾水平的相关研究

摘　　要：目的:探讨急性心肌梗死(AMI)患者早期室性心律失常(VA)与其血钾水平的相关性。方法:回顾性分析120例AMI患者的临床病历资料,搜集患者急诊入院时的基本资料、心肌酶、肌钙蛋白、血钾水平、心电图及入院后24小时内VA发生情况等,根据血钾水平将120例AMI患者分成3组:A组:血钾<3.5mmol/L;B组:3.5mmol/L≤血钾<4.5mmol/L;C组:4.5mmol/L≤血钾<5.5mmol/L。对比观察各组血钾水平及VA得分的情况,并分析血钾水平与VA得分的相关性。结果:A组、B组、C组各组间相比有显著性差异(P<0.05)。Pearson相关分析显示,AMI患者早期血钾水平与室性心律失常的Lown分级呈负相关(r=-0.78,P<0.05)。结论:(1)AMI患者早期室性心律失常的发生与血钾水平相关,血钾水平越低,VA的Lown分级越高;(2)正常低值血钾水平仍有发生VA的风险,保持血钾>4.5mmol/L可能减少VA的发生。Objective：To investigate the correlation of ventricular arrhythmias （VA） and potassium level in the patients with Acute myocardial infarction （AMI）. Methods： According to retrospective analysis of the medical records of 120 patients with AMI, collectted the acute admissions basic information, enzymes, cardiac troponin, serum potassium levels, electrocardiogram and 24 hours after admission, 120 patients with AMI were divided into three groups：group A：serum potassium＆lt;3.5 mmol/L;group B：3.5 mmol/L≤serum potassium＆lt;4.5 mmol/L; groupC： 4.5 mmol/L≤ serum potassium ＆lt;5.5 mmol/L The study of potassium levels and VA score compared , and the correlation between serum potassium level VA score analyzed Results：A group, B group, C group compared among the groups there were significant differences （P〈0.05）. Pearson correlation analysis showed that in patients with AMI early serum potassium levels with VA Lown classification was negatively correlated （r=-0.78, P〈0.05）. Conclusions：（1）The relationship between early VA and potassium level in the patients with AMI were signifaicant, The lower potassium level the higher the VA Lown classification;（2）low-normal serum potassium levels was still the risk of VA, and maintaining serum potassium＆gt;4.5 mmol/L may reduce the occurrence of VA.
