咪康唑鼻窦内灌注联合鼻内镜下手术治疗非侵袭性真菌性鼻窦炎的效果观察

摘　　要：目的探讨咪康唑鼻窦内灌注联合鼻内镜下手术治疗非侵袭性真菌性鼻窦炎的效果。方法将本院2011年12月～2012年12月收治的132例非侵袭性真菌性鼻窦炎患者随机分为对照组和观察组，每组各66例。对照组患者实施常规的鼻内镜下手术治疗，观察组患者实施咪康唑鼻窦内灌注联合鼻内镜下手术治疗，观察两组患者的总有效率、术后愈合时间、术后6个月内的复发情况。结果对照组患者总有效率为81．82％，观察组患者总有效率为96．97％，差异有统计学意义（P〈0．05）。对照组患者术后平均愈合时间（40．2±5．2）d，观察组患者术后平均愈合时间（26．3±3．6）d，差异有统计学意义（P〈0．05）。对照组患者术后6个月内复发20例，观察组患者术后6个月内复发5例，差异有统计学意义（P〈0．05）。结论咪康唑鼻窦内灌注联合鼻内镜下手术治疗非侵袭性真菌性鼻窦炎效果显著，术后伤口愈合快，复发率低，值得临床推广。Objective To explore the effect of of miconazole in sinus perfusion combined with nasal endoscopic surgery in the treatment for non-invasive fungal sinusitis.Methods 132 patients with non-invasive fungal sinusitis admitted into our hospital from December 2011 to December 2012 were randomly divided into control group and observation group,66 cases in each group.Control group patients were treated with regular nasal endoscopic surgery,while observation group patients were treated with miconazole in sinus perfusion combined with nasal endoscopic surgery.The total effective rate,healing time and recurrence after 6 months treatment of two groups were observed.Results The total effective rate of control group was 81.82%,while the total effective rate of observation group was 96.97%,with statistical differenee（P〈O.05）.The average healing time of control group were（40.2_＋5.2）days,the average healing time of observa- tion group were（26.3＋3.6）days,with statistical difference（P〈0.05）.20 cases recurred in the control group while 5 cases in observation group 6 months after operation,with statistical difference（P〈0.05）.Conclusion Effect of miconazole in si- nus perfusion, combined with nasal endoscopic surgery in the treatment for non-invasive fungal sinusitis is obivous, faster postoperative wound healing time,low recurrence rate,it is worthy of expansion in clinic.
