摘　　要：目的:了解我院抗菌药物使用现状,促进我院抗菌药物合理规范化使用。方法:抽取我院2012年各季度抗菌药物处方1 800张,共计7 200张,对不合理处方分类统计,利用Excel软件做出帕累托图,找出最不合理处方的主要因素。将各季度不合理处方进行比较,并加以分析。结果:不合理处方共计2 132张,占抽查处方的29.6%。其中不合理现象主要表现为剂量、疗程、频次不当,用药指征不明确和超适应证用药。从各季度分别计算,我院抗菌药物处方不合理率在逐步下降。结论:需特别加强抗菌药物在用法与用量及诊断书写方面的督导。我院2012年对抗菌药物处方的干预切实有效,达到了进一步规范、合理使用抗菌药物的目的。OBJECTIVE：To investigate the status of antibiotic use in our hospital and promote rational and standard use of antibiotics in our hospital. METHODS ： A total of 7 200 prescriptions （ 1 800 quarterly） sampled in our hospital during 2012 were analyzed statistically with regard to the irrational prescriptions, with Pareto diagram obtained using Excel software with factors associated to the most irrational prescriptions probed and the irrational prescriptions in different quarters were compared analytically. RESULTS： The irrational prescriptions totaled 2 132, accounting for 29.6% of total. The irrational antibiotic use manifested mainly as improper in dosage, course of treatment or DDDs, unclear in indications and off-label use. The proportion of irrational antibiotic prescriptions in each quarter was decreasing gradually. CONCLUSION： It is necessary to tighten supervision on the usage and dosage of antibiotics and writing of diagnosis. The intervention on the antibiotic prescriptions in 2012 was effective, achieving the target of standard and rational use of antibiotics.
