摘 要:目的建立敏感、特异的 RT-nPCR 技术以检测高危人群中HGV 感染状况.方法根据已知并已公认的 HGV 序列,在其5’UTR 区寻找最大同源序列并没计套式引物建立 HGVRNA 的 RT-nPCR 检测法,对129例高危人群(包括48例静脉药瘾者,15例血透患者及各型肝炎患者66例)的血清标本进行检测.结果我们所建立的5’UTR 引物的 RT-nPCR 检测 HGVRNA的敏感性高于 NS3区引物的 RT-nPCR 的敏感性(高10~100倍);IVDUs、血透者及乙型、丙型、非甲戊型肝炎患者HGVRNA 的检出率分别为12%(6/48),13%(2/15)及17%(3/18),22%(7/32),6.2%(1/16),抗-HGV 的检出率分别为10%(5/48),7%(1/15)及22%(4/18),28%(9/32),25%(4/16).输血组的 HGV 感染率(38%)显著高于未输血组(18%)(P<0.05).结论 5’UTR 区引物的 HGVRNA RT-nPCR 检测法敏感、特异;HGV 在高危人群中的感染普遍存在,是部分非甲-戊型肝炎的病因,输血和使用血制品为 HGV 传播的重要的途径.AIM To develop a sensitive and specific reverse transcription-nested polymerase chain reaction(RT-nPCR) and to determine the prevalence of HGV infection in the high risk population. METHODS Sign the two pairs of primers derived from the highly conserved 5'UTR according to the nucleotide homology among three sequences of HGV strains published recently.Set up RT-nPCR to detect HGVRNA in the sera from 129 individuals of high risk population, including 48 intravenous drug users(IVDUs),15 hemodialyzed patients and 66 patients infected with HBV, HCV,and non A-E hepatitis virus. RESULTS The RT-nPCR,developed by us,is 10-100 times more sensitive than that of the RT-nPCR using primers deduced from NS_3 region of HGV genome and the prevalence of HGVRNA in IVDUs,hemodialyzed patients and HBV,HCV,non A-E hepatitis virus infection patients were 12%(96/48),13%(2/15)and 17%(3/18),22(7/32) and 6.2%(1/16),respectively;prevalences of anti-HGV were 10%(5/48),7%(1/15)and 22%(4/18),28%(9/32) and 25%(4/16)respectively.The prevalence of HGV infection in patients with a transfusion history(38%)was significantly higher than that of patients without a transfusion history(18%)(P<0.05). CONCLUSION The RT-nPCR is sensitive and specific in detecting HGVRNA,and HGV infection is common in the high risk population,which indicates that the infection of HGV is one of the reasons for non-A to E hepatitis and that blood transfusion is one of the main routes of HGV transmission.
