血清游离轻链测定在系统性红斑狼疮活动期的应用价值探讨

摘　　要：目的探讨血清游离轻链测定在系统性红斑狼疮(SLE)活动期中的应用价值。方法根据SLEDAI积分将92例SLE患者分为活动期组和非活动期组,检测其血清κ、λ轻链、抗核抗体(ANA)、dsDNA、IgG、IgM、IgA、C3、C4浓度,并与80健康对照进行比较分析。同时进行SLE活动期组ANA滴度和血清游离κ、λ轻链相关性分析。结果 SLE组血清游离κ、λ轻链明显高于健康对照组,SLE组中活动期患者血清游离κ、λ轻链明显高于非活动期患者,差异均有统计学意义(P<0.01)。相关性统计分析显示κ、λ轻链浓度与ANA滴度呈正相关(r=0.993和r=0.983,P<0.05)。结论血清κ、λ游离轻链在诊断SLE活动期有比较高的临床诊断价值。Objective To investigate the clinical value of serum free light chain in systemic lupus erythematosus（SLE） activation period.Methods The 92 SLE patients were divided into activation group and inactivation group according to SLEDAI,serum κ,λ light chain,antinuclear antibody（ANA）,dsDNA,IgG,IgM,IgA,C3,C4 were analysed and compared with 80 healthy controls.ANA titer and serum free light chain were analysed with correlation in SLE activation.Results SLE serum free κ,λ light chain was significantly higher than the control group.Serum free κ,λ light chain in active SLE group was significantly higher than in inactive patients,the difference was statistically significant（P 0.01）.Statistical correlation analysis showed that κ,λ light chain concentrations were positively correlated with ANA titers（r = 0.993 and r = 0.983 P 0.05）.Conclusion Serum κ,λ free light chains have a relatively high value in the diagnosis of active SLE.
