10110例过敏性疾病患者血清过敏原检测与分析

摘　　要：目的通过对舟山地区过敏性疾病患者进行过敏原检测及分析,筛查本地区常见的过敏原。方法采用德国AllergyScreen体外过敏原检测系统检测10110例过敏性疾病患者血清中总IgE和过敏原特异性IgE(SIgE)水平。结果 10110例过敏性疾病患者总IgE阳性率为85.3%,SIgE阳性率为40.6%;食入性过敏原阳性率从高到低依次为:牛奶10.9%、小麦2.4%、鸡蛋白鸡蛋黄2.3%、鱼虾蟹1.8%、腰果花生黄豆0.8%、牛肉羊肉0.4%;吸入性过敏原阳性率从高到低依次为:户尘螨粉尘螨27.9%、蟑螂5.3%、矮豚草蒿2.8%、猫狗毛皮屑2.7%、点青霉分枝孢霉烟曲霉交链孢霉2.2%、柏栎榆梧桐三角叶杨1.4%、葎草1.1%。有24.1%的过敏性疾病患者只对一种过敏原呈阳性反应,有14.5%的过敏性疾病患者对两种过敏原呈阳性反应,也有少数患者对三种及三种以上过敏原呈阳性反应。结论舟山地区吸入性过敏原以户尘螨粉尘螨为主,食入性过敏原以牛奶、小麦为主,通过过敏原筛查可为临床诊断、治疗提供依据,并为患者饮食忌口、避免接触相关过敏原提供帮助。Objective To detect and analyze the allergens in patients with allergic diseases in Zhoushan,so as to find out the common allergens in this area.Methods AllergyScreen in vitro allergens detection system from German was employed to detect the serum levels of total IgE and specific IgE（SIgE） in 10110 patients with allergic diseases.Results In the 10110 patients,the total IgE positive rate was 85.3%,and the SIgE positive rate was 40.6%.The positive rates of food allergen from high to low were milk（10.9%）,wheat（2.4%）,egg white egg yolk（2.3%）,fish,shrimp and crab（1.8%）,cashews,peanuts and soybean（0.8%）,beef and mutton（0.4%）; while those of inhaled allergens were dust mites（27.9%）,roaches（5.3%）,ambrosia artemisiifolia（2.89%）,hair from cat and dog（2.7%）,penicillium notatum,branch spore mould,aspergillus fumigatus and alternaria（2.2%）,cedar oak,Chinese parasol and cottonwood（1.4%）,humulus scandens（1.1%）.In addition,24.1% of the patients showed positive to only one allergen,14.5% showed positive to two allergens,and a small number of them showed positive to three or more than three allergens.Conclusion In Zhoushan area,dust mites were dominant in inhaled allergens,while milk and wheat were dominant in food allergens.Allergens screening can provide basis for clinical diagnosis and treatment of allergic diseases,as well as reminding patients to avoid the related allergens.
