糖化血红蛋白在妊娠期糖尿病筛查中的应用价值

摘　　要：目的探讨糖化血红蛋白（HbA1c）在妊娠期糖尿病（GDM）筛查中的临床价值。方法对GDM患者和正常孕妇各50例,分别检测HbA1c水平,空腹血糖,50 g葡萄糖筛查试验（GCT）和口服75 g葡萄糖耐量试验（OGTT）。使用ROC曲线对GCT和HbA1c作为妊娠期糖尿病的筛查指标进行统计学分析。结果 GDM患者HbA1c水平和GCT均高于正常孕妇,两组有显著性差异,P〈0.01。使用ROC曲线分析,GCT取7.8 mmol/L时,敏感性和特异性分别为68.1%~73.8%和62%~65.3%;HbA1c取5.9%,敏感性和特异性分别为73.2%~80.1%和79.5%~83.1%。结论 HbA1c可以作为妊娠期糖尿病筛查的指标,但需要进一步扩大临床研究,验证切点值。Objective To explore the clinical value of glycosylated hemoglobin（HbA1c） in screening gestational diabetes mellitus（GDM）.Methods GDM patients and normal pregnant women were both selected 50 cases to detect the level of HbA1c,fasting plasma glucose,GCT and OGTT.ROC curve of GCT and HbAlc was used for statistical analysis of gestational diabetes mellitus.Results The values of GCT and HbA1c in GDM group were higher than normal pregnant women,a significant difference was found between the two groups,P0.01.Using the ROC curve analysis,when GCT level was 7.8 mmol/L,the sensitivity and specificity were 68.1%~73.8% and 62%~65.3%; when HbA1c value was 5.9%,the sensitivity and specificity were 73.2%~80.1% and 79.5%~83.1%.Conclusion HbA1c can be used as indicators of gestational diabetes screening,but it need to further expand the clinical studies,verify the cut-off point.
