摘 要:目的研究健康教育干预对于A型性格冠心病(CHD)患者生活质量和1年主要心脏不良事件(MACE)的影响。方法选择281例冠心病患者为研究对象;应用A型行为类型量表分为A型性格和B型性格。将162例A型性格CHD患者分为健康教育干预组和对照组,每组各81人。8周后评价健康教育对于患者身体功能、角色功能、情绪功能、社会功能及总体生活质量的影响,分析健康教育干预与A型性格CHD患者发生MACE的关系。结果 281例CHD组中A型性格162人(57.7%)。健康教育干预8周后,干预组和对照组身体功能、角色功能、情绪功能、社会功能、总体生活质量评分差异均有统计学意义(P<0.001)。随访1年,干预组和对照组分别有21例(25.9%)和28例(34.6%)发生MACE,差异无统计学意义(P>0.05)。结论临床上要重视患者性格类型的评估和干预,从而真正做到对CHD患者的综合、全面的治疗。Objective To evaluate the effect of health education intervention on quality of life (QOL) and major adverse cardiac events (MACE) after 1 year among coronary heart disease (CHD) patients with type A personality. Methods A total of 281 CHD patients were selected as study objects. Type A behavior scale form was applied to determine their personality. 162 CHD patients with type A personality were randomly divided into intervention group (81 persons) and control group (81 persons). The effects evaluation of health education intervention included physical function, role func- tion, emotional function, social function and QOL were evaluated after 8 weeks. The effect of health education on MACE was analyzed by 1-year following-up visit. Results A total of 162 cases (57.7%) were type A personality among 281 CHD patients. After 8 weeks, the scores of physical function, role function, emotional function, social function and QOL in intervention group and control group were different and the difference was significant ( P 〈 0. 001 ). 21 cases in health ed-ucation intervention (25.9%) and 28 cases (34. 6% ) in the control group CHD patients were found MACE after l-year follow-up visit and the difference was not significant (P 〉 0. 05 ). Conclusion It is important to pay attention to patients' personality type evaluation and clinical intervention for CHD patients integrated and comprehensive treatment.
