167例胰十二指肠切除的临床分析

摘　　要：目的总结胰十二指肠切除的经验,分析手术并发症和远期疗效。方法回顾性分析2008年1月至2012年3月172例手术病人。结果胰腺癌的1年、2年、3年生存率分别为66%,35%,19%。无淋巴结转移的胰腺癌相对于有淋巴结转移的具有较好的生存率(P=0.035)胆管下段癌1年、2年、3生存率分别为60%,33%,20%。十二指肠肿瘤1年、2年、3年生存率为91%,64%,61%。结论胰十二指肠在三级医院常成为一种常规手术,无淋巴结转移的胰腺癌术后有着较好的生存率。Objective To summarize the operative complication,fatal rate and long term outcome of pancreaticoduodenectomy（PD）.Methods The clinical data of 167 patients undergoing PD between January 2008 and December 2011 were retrospectively analyzed.Results The 1,2 and 3 survival rate of pancreatic cancer was 66％,35％ and 19％.The long term survival time of lymph node negative pancreatic cancer was significantly higher than the lymph node positive patients（P =0.035）.The 1,2 and 3 survival rate of distal bile duct cancer was 60％,33％ and 20％.The 1,2 and 3 survival rate carcinoma of the duodenum was 91％,64％ and 61％.Conclusion Pancreaticoduodenectomy has become a commonly performed operation in many tertiary care centers.The lymph node negative pancreatic cancer had higher survival time.
