摘 要:目的单用消融电极于二尖瓣下直接标测(不放置冠状窦电极)对35例左侧隐匿性旁道进行射频消融。方法右室心尖起搏下用消融电极沿三尖瓣口标测,确认旁道不在右侧后,将消融电极送至二尖瓣下进行标测和消融。结果 34例左侧隐匿性旁道标测到消融靶点,33例消融成功,1例消融失败,1例复发。与使用冠状窦电极标测相比,消融电极直接标测的X线曝光时间、手术时间均增加。结论单用消融电极可标测和消融左侧隐匿性旁道。Objective To report thirty-five patients with left concealed accessory pathways (LCAP) mapped and ablated using ablation catheter without the help of coronary sinus electrode(CSE). Methods Mapping was performed with abla- tion catheter moving along the tricuspid annular through the way of right ventricle pacing to confirm that the target is not on the right side. Then the ablation catheter was placed under the mitral annulus for mapping and ablation of the pathway. Results Target region was found in 34 case. Thirty-three cases of pathways were ablated successfully. Ablation was failed in 1 case and recurred in another 1 case. The X-ray exposure time and total procedural time were respectively about 10 and 25 minutes more in the group without CSE than in the group with CSE. Conclusion Target mapping and ablation of LCAP using ablation catheter without CSE are feasible and effective.
