摘 要:目的:探讨延迟断脐对婴幼儿贫血的影响及其安全性。方法:将2011年2月至2011年8月在珠海市人民医院足月阴道分娩孕产妇160例按入院单、双日随机分为快速断脐组(n=76)和慢速断脐带组(n=84),快速断脐组在胎儿娩出后5-10 s内断脐,慢速断脐带组在胎儿娩出后2 min断脐,比较两组新生儿胆红素峰值、达峰时间及血红蛋白浓度和血细胞比容。结果:两组新生儿胆红素峰值、达峰时间均无明显差异(P>0.05),而慢速断脐带组新生儿血红蛋白浓度在生后3天及6个月时均显著高于快速断脐组(P<0.05)。结论:慢速断脐在延迟2 min的可控时间内,不提高新生儿胆红素峰值及达峰时间,但可提高新生儿的血容量,增加血红蛋白浓度和血细胞比容,减少缺铁性贫血的发生,且可延续到6个月时。Objective: To explore the effect and the safety of delay fault umbilical cord on infant anemia. Methods: 160 pregnant and lying-in women who matured vaginal delivery by single and double day admission were divided into fast break umbilical group (n=76) and slow changes umbilical group (n=84) from Feb 2011 to Aug 2011. The fetal umbilical cord were broke off navel after childbirth 5 to 10 seconds in fast break umbihcal group and after fetal childbirth 2 minutes in slow changes umbilical group. The bilirubin peak of newborn and peak time, hemoglobin concentration and haematocrit were compared between the two groups. Results:There were no significantly different between the two groups of neonatal bilirubin peak of newborn and peak time (P〉0.05), but the hemoglobin concentrations were significantly higher in slow changes umbilical group than those in fast break umbilical group after childbirth 3 days and 6 months (all P 〈 0.05). Conclusion:Slow umbilical cord broken in 2 minutes controlled time delay neither can increase the neonatal bilirnbin peak and peak time, can also improve the neonatal blood volume, increase hemoglobin concentration and haematocrit, reduce the occurrence of iron deficiency anemia and continuation to six months after the childbirth.
