不同牵引方法对吻合器痔上黏膜环切术治疗重度痔病疗效的影响

摘　　要：目的 比较三种不同的牵引方法对吻合器痔上黏膜环切术（PPH）治疗重度痔病疗效的影响.方法 采用一点牵引法、两点牵引法以及四点牵引法,对120例Ⅲ～Ⅳ度痔行PPH,每种牵引方法40例,观察并记录3种手术方式患者手术时间、术中出血量、术后出血量、肛垫上移率、肛缘水肿时间、黏膜宽度及随访半年后复发情况.结果 所有患者手术过程均顺利,无原发性及继发性大出血.一点牵引法、两点牵引法、四点牵引法手术时间[（32.15±10.20）、（33.02±11.52）、（35.98±9.87） min]、术中出血量[（36.66±14.08）、（35.52±16.25）、（45.88±15.73）ml]、术后出血量[（22.08±7.54）、（15.26±6.55）、（13.02±6.36） ml]、肛垫上移率[80.00％ （32/40）、95.00％（38/40）、92.50％ （37/40）]、黏膜宽度[（2.50±1.25）、（3.05±1.15）、（3.08±1.05） cm]及随访半年后复发情况（3、0、0例）比较差异均有统计学意义（P＜0.05）;而肛缘水肿时间[（3.21±1.05）、（3.32±1.50）、（3.40±1.42）d]比较差异无统计学意义（P＞0.05）.结论 两点牵引法在保证PPH切除痔上黏膜的宽度、完整及均匀的同时,手术时间并未明显增加,能更好地提高手术效果.Objective To compare the clinical effects of 3 different kinds of traction method in procedure for prolapse and hemorrhoids（PPH） in treatment of severe hemorrhoids.Methods One hunderd and twenty cases of Ⅲ-ⅣV degree hemorrhoids were treated by PPH.To observe and record the 3 different kinds of traction method （one point traction method,two point traction method and four point traction method） operation time,intraoperative bleeding,postoperative bleeding,anal cushions move rate,edema of anal edge,mucosa width and recurrence after 6 months.Results All patients during surgery were successful,no primary and secondary bleeding.One point traction method,two point traction method and four point traction method had significant difference in operation time [（32.15 ± 10.20）,（33.02 ± 11.52）,（35.98 ±9.87） min],intraoperative bleeding [（36.66 ± 14.08）,（35.52 ± 16.25）,（45.88 ± 15.73） ml],postoperative bleeding [（22.08 ± 7.54）,（15.26 ± 6.55）,（13.02 ± 6.36） ml],anal cushions move rate [80.00% （32/40）,95.00% （38/40）,92.50% （37/40）],mucosa width [（2.50 ± 1.25）,（3.05 ± 1.15）,（3.08 ± 1.05） cm] and recurrence after 6 months （3,0,0 cases） （P 〈 0.05） ; but 3 different kinds of traction method had no significant difference in edema of anal edge [（3.21 ± 1.05）,（3.32 ± 1.50）,（3.40 ± 1.42） d] （P 〉0.05）.Conclusions Two point traction method in ensuring that the width,completeness and uniformity of the PPH on resection hemorrhoids mucosa,while operative time isn＇t significantly increased,it can better improve surgical results.
