子宫体双侧弓形动脉缝扎术用于剖宫产术中子宫收缩乏力性出血的临床研究

摘　　要：目的 探讨子宫体双侧弓形动脉缝扎术在剖宫产术中子宫收缩乏力性出血中的应用效果.方法 选取2010年1月～2012年12月于解放军第四二五中心医院妇产科行剖宫产术患者80例为研究对象,其中,术中因子宫收缩乏力出血经保守治疗无效而行子宫体双侧弓形动脉缝扎术治疗的40例患者为研究组,经保守治疗无效而采用其他止血方式止血的40例患者为对照组,观察两组患者的出血量、手术时间、即刻止血率、左右子宫动脉RI和S/D等指标.结果 研究组的即刻止血率（95.0％）与对照组（92.5％）比较,差异无统计学意义（P＞0.05）;研究组出血量[（1078.5±46.9） mL]与对照组[（1124.2±44.7）mL]相比,差异无统计学意义（P＞0.05）;研究组手术时间[（61.3±10.6）min]与对照组[（79.5±12.7）min]比,差异有统计学意义（P＜0.05）;研究组左子宫动脉的RI和S/D[（0.78±0.02）、（10.8±3.0）]与对照组[（0.77±0.03）、（11.4±2.7）]相比,研究组[（0.79±0.02）、（11.6±3.2）]右子宫动脉的RI和S/D与对照组[（0.78±0.02）、（12.2±3.1）]相比,差异均无统计学意义（均P＞0.05）.结论 临床中对于剖宫产术中子宫收缩乏力性出血患者应用子宫体双侧弓形动脉缝扎术具有较好的应用效果,止血效果明显,操作也比较简单,同时,对于患者术后子宫与卵巢的功能影响也不大,值得临床中应用与推广.Objective To investigate the application effect of bilateral uterine arcuate artery ligation for cesarean section uterine atony hemorrhage.Methods 80 patients with cesarean section from January 2010 to December 2012 in obstetrics and Gynecology Department of the 425th Central Hospital of Liberation Army were selected as the study object,40 patients of which were treated by bilateral uterine arcuate artery ligation after conservative treatment is invalid as the study group,during the same period,40 patients of which were treated by another ways after conservative treatment is invalid as the control group,the amount of bleeding,operation time,immediate hemostasis rate,uterine artery RI and S/D index of two groups were observed.Results The immediate hemostasis of study group （95.0％） has no statistically significant difference with control group （92.5％） （P ＞ 0.05）.The amount of bleeding of study group and the control group were （1078.5 ± 46.9） mL,（1124.2 ± 44.7） mL,there was no statistically significant difference （P ＞ 0.05）; surgery time of study group and the control group were （61.3 ± 10.6） min,（79.5 ± 12.7） min,the difference of two were statistically significant （P ＜ 0.05）; the left uterine artery RI and S/D of the control and study groups were （0.77 ± 0.03）,（11.4 ± 2.7） and （0.78 ± 0.02）,（10.8 ± 3.0）; the right uterine artery RI and S/D of controland study groups were （0.78 ± 0.02）,（12.2 ± 3.1） and （0.79 ± 0.02）,（11.6 ± 3.2）,there were no statistically significant difference （all P ＞ 0.05）.Conclusion There is a good clinical application effect of cesarean section uterine atony bleeding patients with uterus bilateral arcuate artery ligation,and has an obvious hemostatic effect,the operation is relative simple,at the same time,there is little influence on the function of uterine and ovarian after operation,it is worthy of clinical application.
