中心静脉导管胸腔置入闭式引流在呼吸内科临床中的应用

摘　　要：目的探讨中心静脉导管闭式引流对于胸腔积液及自发性气胸的治疗效果,分析其临床价值。方法选取2011年6月-2012年6月在某科治疗的胸腔积液患者及自发性气胸患者各20例,分为胸腔积液组和气胸组,均采用中心静脉导管植入胸腔治疗,并选取同期采用传统治疗的胸腔积液及自发性气胸患者各20例作为对照组,分别比较中心静脉导管闭式引流与传统治疗方式在胸腔积液或自发性气胸中的效果。结果 (1)中心静脉导管置入治疗胸腔积液效果优于传统方式,但两者治疗自发性气胸效果相似;(2)相对于传统治疗方式,中心静脉导管置入治疗胸腔积液或者自发性气胸并发症更低(P﹤0.05)。结论中心静脉导管对于胸腔积液或者自发性气胸的疗效肯定,安全性高,可显著降低操作相关并发症。OBJECTIVE The aim of the study was to assess the curative effect of closed thoracic drainage by installation of a centeral venous catheter in the treatment of pleural effusion and spontaneous pneumothorax, and to analyze the clinical value of the technique. METHODS 20 pleural effusion patients and 20 spontaneous pneumothorax patients treated in our hospital between June of 2011 and June of 2012 were selected and divided into a pleural effusion group and a spontaneous pneumotho- rax group; both groups were treated by installation of a central venous catheter. Meanwhile, 20 pleural effusion patients and 20 spontaneous pneumothorax patients treated by conventional therapies during the same period were selected as the control group. The effectiveness of closed thoracic drainage using a centeral venous catheter and conventional therapies in the treat- ment of pleural effusion and spontaneous pneumothorax were compared and analyzed. RESULTS （ 1 ） Installation of a center- al venous catheter was more effective than the conventional methods in treating pleural effusion, but had similar effects on spontaneous pneumothorax as the conventional method. （2） As compared with the conventional methods, installation of a central venous catheter resulted in fewer complications of pleural effusion and spontaneous pneumothorax. CONCLUSION Central venous catheter is effective in treating pleural effusion and spontaneous pneumothorax, high in safety, and can signif- icantly reduce the incidence of complications associated with the operations.
