摘 要:目的 探讨钾离子反复刺激大鼠膀胱组织增加大鼠膀胱逼尿肌收缩敏感性与RhoA/Rho激酶途径的相关性.方法 雌性SD大鼠20只,随机分为刺激组和对照组,每组10只.刺激组:膀胱内灌注硫酸鱼精蛋白(10 g/L)及氯化钾溶液(500 mmol/L),每3d灌注1次,共20次,为期2个月.对照组:同样方法灌注生理盐水.2个月后两组分别行膀胱尿动力学检查.取两组大鼠膀胱逼尿肌组织,用于观察预收缩后Y-27632对大鼠膀胱逼尿肌的舒张作用.结果 刺激组膀胱排尿间隔时间及膀胱容量均小于对照组[(256±51)s比(566 ±54)s;(226±60)μl比(394±37)μl,P<0.01]、储尿期膀胱收缩次数、膀胱排尿压力阈值均大于对照组[(2.32±0.45)次/分比(0.86±0.27)次/分;(9.39±1.22) mm Hg(1 mm Hg=0.133 kPa)比(6.03 ±0.81) mmHg,P<0.01].刺激组膀胱逼尿肌对Y-27632的敏感性较对照组明显增加,最大舒张率明显增大[(71.5±2.8)%比(45.1±3.1)%,P<0.01].结论 钾离子反复刺激大鼠膀胱组织可增加大鼠膀胱逼尿肌收缩敏感性,其作用机制可能与RhoA/Rho激酶途径有关,从而推断间质性膀胱炎发病可能与钾离子通过RhoA/Rho激酶途径增加膀胱逼尿肌收缩敏感性有关.Objective To investigate the relationship between RhoA/Rho kinase pathway and the increase of bladder detrusor contraction in rats stimulated repeatedly by potassium.Methods A total of 20 female SD rats were randomly divided into two groups:experiment group and control group (n =10 each).Intravesical protamine sulfate (10 g/L) and potassium chloride (500 mmol/L) were administered to experiment group once every 3 d for 2 months.Normal saline was used for controls.Urodynamic examinations were conducted subsequently in both groups after 2 months.Bladder detrusor strips isolated from both two groups were first precontracted,then relaxant effects of Y-27632 were observed.Results The bladder intercontraction interval and bladder capacity were significantly reduced [(256 ± 51) s vs.(566 ± 54) s,and (226 ±60) μl vs.(394 ±37) μl,P <0.01],and the bladder nonvoiding contractions and pressure threshold were obviously increased [(2.32 ± 0.45) times/min vs.(0.86 ± 0.27) times/min,(9.39 ± 1.2) mm Hg (1 mm Hg=0.133 kPa) vs.(6.03 ±0.81) mm Hg,P<0.01] in experiment group as compared with control group.As compared with control group,the sensitivity and maximum relaxant effect of Y-27632 were increased significantly in experiment group [(71.5 ± 2.8)% vs.(45.1 ± 3.1)%,P < 0.01].Conclusion Repeated stimulation of rat bladder tissues by potassium can increase the contraction sensitivity of bladder detrusor,which may be related to the RhoA/Rho kinase pathway.It is deduced that potassium may increase the contractility of bladder detrusor through the RhoA/Rho kinase pathway,leading to the occurrence of interstitial cystitis.
