摘  要:目的调查金黄色葡萄球菌的耐药与毒力遗传学背景,为临床治疗提供参考依据。方法收集2013年1-5月临床分离到的55株金黄色葡萄球菌,用聚合酶链反应(PCR)的方法分析mecA、sasX、pvl、tst基因。结果共55株金黄色葡萄球菌经mecA基因检测阳性35株,即该35株为MRSA,另20株为MSSA;sasX基因阳性1株,阳性率1.8%;pvl基因阳性41株,阳性率74.5%;tst基因阳性0株;MRSA与MSSA的sasX、pvl、tst基因检出率差异无统计学意义;MRSA与MSSA对左氧氟沙星、阿奇霉素、四环素耐药率分别为65.71%、35.00%、82.86%与40.00%、57.14%、25.00%,差异有统计学意义(P<0.05)。结论 MRSA因携带mecA基因,对β-内酰胺类、氨基糖苷类、大环内酯类、四环素类药物的耐药率要高于MSSA,对MRSA与MSSA进行sasX、pvl、tst基因检测尚为国内外首次;该组菌株无论MRSA还是MSSA,pvl基因高检出率为特征。OBJECTIVE To investigate the genetic background of resistance and virulence in a group of Staphylococcus aureus strains so as to provide evidence for the clinical data. METHODS Totally 55 strains of S. aureus isolated from Jan to May 2013 were collected, then the mecA, sasX, pvl, and tst genes were analyzed by means of the polymerase-chain-reaction (PCR). RESULTS Of totally 55 strains of S. aureus, 35 strains were tested positive for mecA, which were identified as MRSA, and the rest of 20 strains were MSSA; 1 strain was tested positive for sasX with the positive rate of 1. 8%; the pvl gene was tested positive in 41 strains with the positive rate of 74.5% the tst gene was tested negative. The difference in the detection rate of the sasX, pvl, or tst gene between the MRSA and MSSA was not statistically significant. The drug' resistance rates of the MRSA and MSSA to levofloxacin, azithromycin, and tetracycline were 65. 71%, 35. 00%, 82. 86%,40. 00%, 57. 14%, and 25.00%, respectively, the differences were statistically significant (P〈0.05). CONCLUSION The MRSA strain carries the mecA genes the drug resistance rates of the MRSA to β-lactams, aminoglycosides, macrolides, and tetracyclines are higher than those of the MSSA strain; it is for the first time to detect the sasX, pvl, or tst gene in the MRSA and MSSA in the world; for this group of strains, both the MRSA and MSSA are characterized by the high detection rate of pvl gene.
